The answer to that question has to come not from a single RCT, but from large, pooled meta-analyses covering millions of participants observed over years, ideally decades.

Fortunately, such meta-analyses exist, and in considerable numbers — the Mediterranean diet has been the subject of epidemiological research since the 1960s, when researcher Ancel Keys observed in his famous Seven Countries Study that residents of Crete and southern Italy had markedly lower cardiovascular mortality than residents of Northern Europe or the United States, despite comparable or higher total fat intake. That discovery launched decades of research, culminating today in vast, multi-million-participant meta-analyses — we break down two of them below.

A sample size rarely seen in nutrition science

Strong evidence

A sample exceeding 4 million participants is a scale rarely encountered in dietary research — for comparison, many widely publicized nutrition studies rest on samples of a few thousand to a dozen or so thousand people. Such a large sample significantly increases the precision of estimates (note the narrow confidence intervals around each result) and reduces the risk that the observed effect is a product of chance. That doesn't eliminate the fundamental limitation of this type of research, though: it's still observational data, not an experiment, so it doesn't prove direct causality as unambiguously as a randomized clinical trial like PREDIMED.

What a study focused on older adults shows

The Sofi et al. meta-analysis covers a broad, age-diverse population, but a separate question is whether the same benefits apply to older adults, whose disease mechanisms, metabolism, and overall health status already differ from younger adults'. The answer comes from a study by Bonaccio and colleagues, based on the Italian Moli-sani cohort — one of the largest, longest-running research projects on lifestyle and health in the Italian population.

The Moli-sani cohort study followed 5,200 people aged 65 and older for a median of 8.1 years, during which 900 deaths were recorded. Each one-point increase on the Mediterranean diet score was associated with a 6% lower risk of all-cause death (HR=0.94; 95% CI 0.90-0.98), a 9% lower risk of cardiovascular death (HR=0.91; 95% CI 0.83-0.99), and an 11% lower risk of death from causes other than cardiovascular disease and cancer (HR=0.89; 95% CI 0.81-0.96). The accompanying meta-analysis of seven prospective studies (11,738 participants, 3,874 deaths) confirmed a consistent direction of effect: each point of higher diet adherence was associated with a 5% lower risk of death (95% CI 4-7%), and the relationship between diet adherence and death risk was linear — higher adherence meant stronger protection, with no clear saturation threshold.

The effect holds even in advanced age

Strong evidence

This is an important complement to the Sofi meta-analysis: the benefit of following a Mediterranean diet isn't limited to middle-aged people, who typically have the longest time horizon for prevention to work. Even in people over 65, followed for a relatively short period by longevity-research standards (an average of 8 years), the relationship between diet and survival remained statistically significant and directionally consistent with the larger, population-wide meta-analysis. This suggests it's never "too late" to start benefiting from this eating pattern, though of course the longer the period of consistent adherence, the greater the cumulative benefit.

Why the numbers from the two studies differ slightly — and why that's not a problem

A careful reader will notice that the mortality risk reduction in the Sofi meta-analysis (8% per 2 points, roughly 4% per point) and in the Bonaccio study (5-6% per point) aren't identical. That's a normal, expected phenomenon when comparing independent studies, stemming from differences in populations (age, country of origin, follow-up period), the diet-adherence scales used (not every study measures "Mediterranean diet adherence" in exactly the same way), and the statistical methods used to control for other risk factors.

Since the numbers from different studies differ, none of them can be trusted, and the topic is essentially scientifically unclear.

A discrepancy of a few percentage points between independent studies isn't a sign of uncertainty about the whole topic — it's the natural methodological variation researchers work with every day. Far more important than the exact numerical value is that the direction and order of magnitude of the effect converge in both cases — stronger adherence to the Mediterranean diet consistently correlates with lower death risk, in the general population and among seniors, across different countries and different decades of observation. This consistency across independent research teams, using different methodologies on different populations, is precisely what nutritional epidemiology considers a strong signal of a real effect, not an artifact of any single study.

It's also worth noting that in the Bonaccio study, the dose-response relationship was linear, with no visible saturation threshold within the range studied — meaning the benefit rises proportionally with the degree of diet adherence, rather than there being some minimum threshold beyond which moving further toward the Mediterranean pattern stops paying off. That's practically important information: even a partial implementation of this eating pattern (more vegetables, swapping butter for olive oil, more fish instead of red meat) should bring a proportional, if of course smaller than full implementation, benefit.

What's actually behind these numbers — mechanisms, not just statistics

The numbers from meta-analyses, though convincing on their own, gain additional credibility when accompanied by a consistent, biologically plausible mechanism — and in the case of the Mediterranean diet, such a mechanism is well documented and multi-pronged, not reducible to a single, isolated ingredient. High fiber and polyphenol intake supports gut microbiome diversity and reduces chronic low-grade inflammation — a process underlying not just cardiovascular disease, but also some cancers, neurodegenerative disease, and the general pace of bodily aging. Monounsaturated fats from olive oil improve lipid profile and vascular endothelial function, omega-3 fatty acids from fish compete with pro-inflammatory arachidonic acid for the same metabolic enzymes, and limiting red meat and processed food reduces exposure to compounds linked in studies to elevated colorectal cancer risk. No single one of these mechanisms fully explains the observed reduction in all-cause mortality on its own — it's precisely their simultaneous, cumulative action across many independent causes of death that best fits what the large cohort meta-analyses show.

Limitations worth keeping in mind

What this data doesn't prove

Both papers discussed are observational studies (cohort meta-analyses), not randomized clinical trials — despite the enormous sample sizes, the influence of confounding factors, such as overall socioeconomic status, physical activity level, or access to healthcare, which may correlate with both Mediterranean diet adherence and lifespan independently of the diet itself, can't be fully ruled out. Full, direct confirmation of causality for all-cause mortality would require a randomized trial lasting decades, which is in practice extremely difficult to conduct — which is why large, consistent cohort meta-analyses remain the best available evidence for this particular question.
