We gathered ten ways with real scientific documentation, with a clear assessment of how much LDL reduction you can realistically expect from each.

LDL cholesterol: a silent risk factor you can still influence

LDL cholesterol (low-density lipoprotein) transports cholesterol from the liver to tissues, and in excess it deposits in artery walls, initiating an atherosclerotic process that can proceed for years without any symptoms. That's exactly why a lipid panel, not how you feel, is the only reliable way to detect elevated LDL — we describe exactly what this result shows and how to interpret it in our knowledge-base entry on the lipid panel.

The problem with popular "natural ways to lower cholesterol" lists is similar to what we've already described for other metabolic topics on this site: they treat very different interventions as equally effective as long as they sound natural. We've already directly tested several of these popular recommendations — for example, we separately tested garlic and cinnamon for their real effect on LDL, with very different final results for each.

In this article, each of the ten ways is described with its real evidence strength and an approximate effect size from randomized-trial meta-analyses. 1. Soluble fiber — oats, psyllium husk, legumes

Soluble fiber, especially the beta-glucan found in oats and barley and the mucilage from psyllium seeds, binds bile acids in the gut, forcing the liver to use more circulating cholesterol to produce new bile acids — this directly lowers the pool of LDL cholesterol in the blood. It's one of the few dietary approaches on this list with evidence at the level of large meta-analyses, not just small individual studies.

A meta-analysis of 28 randomized trials compared consuming at least 3 g of oat beta-glucan daily against control groups. Beta-glucan lowered LDL by 0.25 mmol/L (about 10 mg/dL) and total cholesterol by 0.30 mmol/L, with no significant effect on HDL or triglycerides. The effect was consistent across different study subgroups and populations.

In practice, 3 g of beta-glucan is roughly 40 g of oatmeal daily (one solid bowl of oatmeal) or an equivalent serving of oat bran — a realistic dose to introduce permanently, unlike many supplements requiring far more raw material than anyone realistically eats.

2. Plant sterols and stanols (fortified margarines and yogurts)

Plant sterols and stanols have a chemical structure similar to cholesterol, so they compete with it for absorption in the gut, limiting how much dietary cholesterol enters the bloodstream. It's one of the best-documented, most measurable dietary ways to lower LDL — hence their widespread presence as an additive in "cholesterol-lowering" margarines and yogurts sold in most supermarkets.

A meta-analysis of 124 clinical trials (over 9,600 participants) found a dose-dependent LDL reduction of 6-12% with an intake of 0.6-3.3 g of plant sterols or stanols daily, with the effect curve flattening above roughly 3 g/day. The effect held regardless of whether the sterols were added to margarine, yogurt, or another carrier.

Plant sterols and statin therapy

Plant sterols and stanols work through a different mechanism than statins (they limit cholesterol absorption from food rather than inhibiting its synthesis in the liver), so the effects of the two interventions are additive. People on statin therapy — whose action and safety profile we describe in our entry on statins — can achieve an additional LDL reduction by adding sterol-fortified foods to their diet, after consulting their doctor.

3. Swapping saturated fats for unsaturated ones

Replacing saturated fats (butter, lard, fatty meat, coconut oil) with mono- and polyunsaturated fats (olive oil, vegetable oils, nuts, fatty fish) is one of the best-established dietary recommendations in preventive cardiology — not as a single, new discovery, but as a conclusion drawn from decades of combined interventional and observational research.

The effect size depends on the size of the swap, not just its direction

Strong evidence

A classic meta-analysis by Mensink, Zock, Kester, and Katan (2003, American Journal of Clinical Nutrition, PMID 12716665), covering 60 controlled trials, confirmed that replacing saturated fats with unsaturated fats significantly lowers LDL, while replacing them with refined carbohydrates alone does not produce this effect. How much LDL drops depends on how large a share of the diet is actually swapped — the larger the shift of calories from saturated to unsaturated fat, the greater the LDL reduction, typically in the range of several to low double-digit percent with a realistic, typical dietary change.

4. Regular aerobic activity

Regular aerobic exercise lowers LDL partly directly (by increasing the activity of enzymes involved in lipoprotein metabolism), and partly indirectly — through fat loss and improved insulin sensitivity, which themselves affect the lipid profile. The effect is usually more modest than with drug therapy, but clearly present and, unlike medication, comes with additional cardiovascular benefits unrelated solely to LDL itself.

A meta-analysis of randomized trials combining regular aerobic training with a prudent diet found an average LDL reduction of 9.2 mg/dL (about 7.2%) compared with control groups. The effect was consistent across different adult populations, though the size of the reduction depended on training intensity and volume as well as concurrent dietary changes.

5. Losing excess body weight

Losing excess body weight lowers LDL, though — worth stating honestly — the effect on LDL tends to be more modest and less consistent across studies than the same weight loss's effect on triglycerides or HDL, which react much more clearly to changes in body weight. This doesn't mean weight loss is pointless for LDL — it means that for someone with elevated LDL but a normal weight, weight loss on its own isn't the main tool to reach for, unlike points 1-4 above.

In people with overweight or obesity, the mechanism partly overlaps with point 4 — weight loss usually goes hand in hand with dietary change and increased physical activity, so in practice it's hard to separate the pure effect of "losing weight" from the effect of the changes that led to it. In clinical practice, weight loss is usually treated as a side effect of a healthier diet and activity level, rather than a separate target for LDL specifically.

6. Nuts — a handful daily instead of a highly processed snack

Nuts (almonds, walnuts, hazelnuts, pistachios) are rich in unsaturated fats, fiber, and plant sterols, which gives them a combined, multi-pathway effect on LDL — partly by displacing less favorable snacks from the diet, and partly through their components' direct effect on cholesterol absorption and metabolism.

A meta-analysis and dose-response analysis of 61 interventional trials (2,582 participants) found that tree nut consumption lowered LDL by an average of 4.8 mg/dL per daily serving, with a clearer effect at an intake of at least 60 g daily. The dose-response relationship was nonlinear — the largest gain in benefit was seen going from no nuts to a moderate portion.

Nuts are calorie-dense, so they work best as a substitute for another snack (e.g., sweets or chips) rather than an addition to an otherwise unchanged diet — in the latter case, the calorie surplus can offset the lipid benefit through weight gain over the long run.

7. Garlic

Garlic has one of the largest dietary meta-analyses behind it when it comes to cholesterol research — we tested this topic in detail in a dedicated article. In short: a large meta-analysis of 39 randomized trials found a real, but modest, LDL reduction with regular, long-term (at least 2 months) garlic intake, clearly smaller than the effect of drug therapy and dependent on form (fresh garlic, powder, aged extract) and duration of the intervention.

8. Cinnamon — popular, but with an honest caveat

Cinnamon lands on nearly every "natural ways to lower cholesterol" list, but when it comes to LDL specifically, the evidence is much weaker than this spice's popular reputation suggests — we tested this directly in a dedicated article on our site.

Myth

Cinnamon lowers LDL cholesterol by a similar mechanism to how it improves glycemic control in insulin resistance.

Fact

A 2017 meta-analysis of randomized trials found that cinnamon supplementation significantly lowered triglycerides and total cholesterol, but showed no statistically significant effect on LDL. That's an honest, "null" result for this specific parameter — so cinnamon isn't worth treating as a way to lower LDL, though it may have other metabolic benefits, described in our article on cinnamon and LDL cholesterol.

9. The overall Mediterranean diet pattern

Instead of introducing points 1-6 as separate, disconnected tactics, you can adopt an overall eating pattern that combines most of them at once — olive oil as the main fat source, legumes and whole grains rich in fiber, nuts, fish instead of red meat. The Mediterranean diet is one of the best-studied eating patterns in preventive cardiology, with documented effects not only on LDL but also on hard endpoints such as all-cause mortality.

10. Cutting back on simple sugars and refined carbohydrates

Excess simple sugars and highly processed carbohydrates affect the lipid profile more subtly than saturated fat — often not directly raising LDL concentration, but promoting the formation of so-called small, dense LDL particles, which are more atherogenic (more strongly promote atherosclerosis) than standard, larger LDL particles at the same total cholesterol concentration in that fraction. This distinction rarely shows up on a standard lipid panel, which reports only LDL concentration, not particle size.

An indirect but real effect

Moderate evidence

A diet rich in simple sugars and refined carbohydrates also raises triglycerides and lowers HDL, worsening the entire lipid profile even with unchanged LDL cholesterol. Evidence for this mechanism comes mainly from research on metabolic syndrome and insulin resistance, where the effect is well documented, though the data specifically on LDL (rather than the whole lipid profile) are somewhat weaker than for points 1-4.

What these ways won't replace

When diet and lifestyle aren't enough

For some people — especially those with familial hypercholesterolemia, very high baseline LDL, or already-diagnosed cardiovascular disease — lifestyle changes alone won't bring LDL into a safe range, and drug therapy (most often statins) is necessary and shouldn't be delayed while waiting for diet to work. None of the interventions described here should be treated as a reason to stop prescribed medication on your own — a decision to modify treatment is made solely by the treating physician, ideally based on a repeat lipid panel after a few months of change.

10 ways to lower LDL naturally — at a glance

Our editorial recommendation

The strongest, most consistent evidence among these ten belongs to four interventions: soluble fiber, plant sterols, swapping saturated for unsaturated fats, and regular aerobic activity — and they work best together, not separately, because they partly complement each other (fiber and sterols limit cholesterol absorption, the fat swap changes what the liver synthesizes, physical activity improves overall lipid metabolism). Nuts, garlic, and the overall Mediterranean diet are a sensible, well-documented addition. Cinnamon, despite its popularity, is worth skipping specifically in the context of LDL — an honest conclusion from the data, even if less satisfying than a supplement label's promise.

No single spice or superfood will replace what these do together: less saturated fat, more fiber, regular movement and, when needed, medication. The goal isn't to find one magic ingredient, but to assemble these pieces into something you can realistically stick with for years, not two weeks.

With consistent implementation of several of the described ways at once (fiber, plant sterols, swapping fats, physical activity), a realistic LDL reduction usually falls in the range of 15-25%, though for a specific person it depends on baseline level, genetics, and how rigorously the changes are implemented. That's substantial, but with a very high baseline LDL, it's often still not enough to avoid drug therapy.

The largest available meta-analysis of prospective cohorts (over 1.7 million person-years of follow-up) found no significant association between moderate egg consumption and cardiovascular risk in metabolically healthy people — we describe the full data in our article on eggs and blood cholesterol. For most people, eggs aren't the main problem to address if LDL is elevated.

Most dietary and activity-related effects stabilize after 6-8 weeks of consistent practice, so it's reasonable to repeat a lipid panel no earlier than 2-3 months after starting changes, rather than after two weeks — an earlier measurement may not reflect the full effect.

They aren't a safe substitute — monacolin K is a naturally occurring compound chemically identical to lovastatin (a statin drug), so it carries the same potential side effects and interactions as statins, with much less controlled, variable active-ingredient content in the supplement. Usually not to the same degree — familial hypercholesterolemia results from a genetic defect in the LDL receptor, so diet and lifestyle alone rarely bring LDL into a safe range. In these people, lifestyle changes remain a valuable addition, but drug therapy is usually necessary from the start, not a last resort after an unsuccessful diet-only attempt.

It depends on the brewing method — unfiltered coffee (e.g., Turkish, French press, or Scandinavian boiled coffee) contains diterpenes (cafestol and kahweol), which can raise LDL with large, regular consumption, while filtered coffee (e.g., from a drip machine) contains far less of them and doesn't show this effect at a comparable scale.

No — that's a popular myth. Moderate alcohol consumption is sometimes linked in observational studies to slightly higher HDL, but there's no good evidence for alcohol lowering LDL, and excessive consumption clearly worsens the entire lipid profile, including raising triglycerides. Starting to drink alcohol to improve a lipid panel has no basis in the data.

Indirectly, yes — chronic stress and sleep deprivation favor worse food choices, less physical activity, and greater visceral fat gain, which secondarily worsens the lipid profile. The direct, independent effect of stress or sleep itself on LDL (apart from these indirect factors) is less well documented than for triglycerides or insulin sensitivity.
