Eggs and Blood Cholesterol: Does an Egg-Rich Diet Harm Your Heart?

For decades, eggs served as the poster child for a food to "avoid" if you cared about your heart — because of the high cholesterol content in the yolk. The newest, massive analysis combining three prospective US cohorts (215,618 people, followed for up to 32 years) and an updated meta-analysis spanning 1.72 million participants worldwide paints a far calmer picture: eating up to one egg a day shows no significant association with elevated cardiovascular risk. In Asian cohorts, researchers even observed a mild protective effect. This is one of those cases where the scale and quality of newer data shifts conclusions from decades past — not because the old studies were poorly done, but because newer analyses have the statistical power to catch nuances smaller samples couldn't see. In this article, we look at exactly what the numbers show, where certainty ends, and where nuance begins.

Where the egg-and-cholesterol myth came from

One large chicken egg contains around 180-200 mg of cholesterol, almost entirely in the yolk — more than in most single foods. For decades, dietary guidelines in many countries restricted egg intake for exactly this reason, relying on a simplified model: dietary cholesterol raises blood cholesterol, which in turn raises cardiovascular risk. That causal chain turned out to be much weaker than assumed.

In most people, the liver regulates its own cholesterol production in response to the amount consumed through food — this mechanism partly compensates for dietary cholesterol intake, which explains why the link between eating eggs and blood cholesterol levels is much weaker in population studies than intuition might suggest.

The starting point is the 2020 work by Drouin-Chartier and colleagues, published in the British Medical Journal — a combination of three large US prospective cohort studies with a systematic review and updated meta-analysis of global data.

Analysis of three cohorts: the Nurses' Health Study (83,349 women), Nurses' Health Study II (90,214 women), and Health Professionals Follow-up Study (42,055 men) — 215,618 people total, followed for up to 32 years. Eating at least 1 egg/day versus less than 1 egg/month was associated with HR=0.93 (95% CI 0.82-1.05) for cardiovascular disease — not statistically significant. The updated meta-analysis of 33 risk estimates covered 1,720,108 participants and 139,195 cardiovascular events: each additional egg/day was associated with a pooled RR=0.98 (95% CI 0.93-1.03; I²=62.3%). Separately for coronary heart disease: RR=0.96 (0.91-1.03); for stroke: RR=0.99 (0.91-1.07). Asian cohorts showed an inverse association: RR=0.92 (0.85-0.99).

Strong evidence

The scale of this analysis is key — with nearly 1.72 million participants and over 139,000 cardiovascular events, if egg consumption truly raised risk significantly, a study of this statistical power would practically have to catch it. The fact that the confidence interval for the overall result (0.93-1.03) spans 1.0 means there's no evidence of harm at moderate intake.

Why the Asian cohort result differs

An interesting element of this meta-analysis is the divergence between Western and Asian cohorts — the latter showed an inverse, protective association (RR=0.92). The study authors don't definitively resolve the cause of this difference, but a plausible explanation is dietary context: in populations where eggs replace highly processed foods or refined carbohydrates in the diet, their net effect on cardiovascular health may be favorable precisely because of what they replace, not because of the eggs themselves.

Heterogeneity in the meta-analysis results (I²=62.3%) points to meaningful differences between component studies, which may reflect differences in populations, intake measurement methods, or dietary context. Results concern the general population — they shouldn't automatically be extended to people with already diagnosed cardiovascular disease or lipid disorders, for whom separate studies may yield different conclusions.
