Overview

The Mediterranean diet is based on the traditional eating pattern of people living around the Mediterranean Sea in the mid-20th century — mainly Greece (Crete) and southern Italy, before the region adopted the Western industrial diet. Its key elements are a large amount of vegetables, fruit, olive oil as the main fat source, fish and seafood, legumes, nuts, and whole-grain products, moderate consumption of dairy (mainly fermented — yogurt, cheese) and wine with meals, alongside limited consumption of red meat and processed food.

It's the best-studied eating pattern in the history of nutrition science — encompassing dozens of large cohort and randomized studies, including the landmark PREDIMED trial, one of the first to provide hard causal (not just correlational) evidence for the cardiovascular benefits of a specific eating pattern.

It's one of the few eating patterns with no significant health contraindications — it works both as cardiovascular prevention and as support for cognitive health in older age. The one notable caution concerns people on warfarin therapy — high intake of olive oil and green leafy vegetables rich in vitamin K may require adjusting the medication dose under medical supervision. The real challenge tends to be adherence: the pattern requires regular cooking and access to fresh vegetables, fish, and good-quality extra virgin olive oil, so it benefits least the people expecting quick, one-off results or unwilling to make a lasting change to their cooking habits — the benefits shown in the PREDIMED trial resulted from years of consistent use, not short-term trials. Wine isn't a necessary element — the key benefits come from vegetables, olive oil, and fish, and people who don't drink alcohol shouldn't start drinking for health reasons.

Where it's found

The pattern originates from the traditional cuisine of the Mediterranean region — primarily Greek Crete and southern Italy — from the immediate postwar period, before these countries underwent dietary industrialization. It was these populations, alongside other Mediterranean countries, that served as the reference point in comparative diet studies worldwide.

History of use

Scientific interest in the Mediterranean diet was sparked by the American physiologist Ancel Keys, who, as part of the Seven Countries Study (begun in the 1950s), noticed exceptionally low cardiovascular mortality in Crete and southern Italy despite relatively high fat intake. This observation launched decades of cohort studies, culminating in 2018 with the PREDIMED trial — one of the first large randomized trials to provide hard causal, not just correlational, evidence for the cardiovascular benefits of this eating pattern.

Mechanism of action

A high content of monounsaturated fatty acids (mainly from olive oil), fiber, polyphenols (including from olives, red wine, vegetables), and omega-3 fatty acids (from fish) supports the lipid profile by lowering LDL and raising HDL, reduces vascular inflammation (lower CRP and other inflammatory markers), and favorably affects gut microbiome diversity thanks to the high intake of fermentable fiber from vegetables and legumes.

At the biochemical level, omega-3 fatty acids from fish (EPA and DHA) compete with arachidonic acid for the same enzymes (cyclooxygenases and lipoxygenases), shifting eicosanoid production toward less pro-inflammatory variants, and polyphenols from extra virgin olive oil (including oleocanthal) show, in in-vitro studies, an inhibitory effect on the same enzymes as non-steroidal anti-inflammatory drugs — though to a much milder degree. Monounsaturated fatty acids also increase LDL particles' resistance to oxidation, which, alongside improved vascular endothelial function (greater nitric oxide bioavailability), explains part of the cardioprotective effect observed in the PREDIMED trial.
