Overview
Statins are a class of drugs that inhibit the enzyme HMG-CoA reductase, key to hepatic cholesterol synthesis, thereby lowering LDL cholesterol levels in the blood. They are one of the most thoroughly studied drug groups in all of medicine — the Cholesterol Treatment Trialists' Collaboration meta-analysis, covering data from dozens of randomized trials and hundreds of thousands of patients, clearly showed that lowering LDL by each unit translates into a proportional reduction in the risk of major cardiovascular events.
Despite this solid evidence base, statins are the subject of numerous fears, partly driven by rare side effects that receive outsized media attention. The actual incidence of serious adverse effects (such as significant muscle damage) is, in placebo-controlled trials, considerably lower than subjective reports under open-label use would suggest — a phenomenon called the nocebo effect, where mere awareness of taking a statin increases the reporting of muscle complaints regardless of the drug's actual action.
Who can genuinely benefit from this? People with elevated cardiovascular risk for whom lifestyle changes alone proved insufficient to reach their target LDL level, and people who have had a prior cardiovascular event (secondary prevention), where the benefits of statins are especially well documented.
Mechanism of action
Statins competitively inhibit HMG-CoA reductase, the enzyme catalyzing the rate-limiting step in intracellular cholesterol synthesis in the liver. Lowering intracellular cholesterol triggers a compensatory increase in the number of LDL receptors on the surface of hepatocytes, increasing uptake of circulating LDL from the bloodstream into the liver and lowering its serum concentration.
Beyond lowering LDL, statins also show pleiotropic effects independent of cholesterol itself — they stabilize atherosclerotic plaques, reduce inflammation in the vessel wall, and improve endothelial function, which partly explains why clinical benefits appear faster than the pace of lipid profile change alone would suggest.
