Overview

DASH (Dietary Approaches to Stop Hypertension) is an eating pattern developed from the ground up as a blood-pressure-lowering intervention, not a general 'healthy eating' model adapted for that purpose later. It emphasizes vegetables, fruit, whole grains, low-fat dairy, poultry, fish, and legumes, while limiting sodium, red meat, sweets, and saturated fat.

It's one of the few eating patterns tested in large, multicenter randomized trials funded directly by the U.S. National Institutes of Health — the DASH trial (1997) and its extension, DASH-Sodium (2001), provided hard evidence that dietary change alone can lower blood pressure to a degree comparable to some first-line medications in people with mild hypertension.

Who can realistically benefit, and who probably can't? The largest, most measurable benefits go to people with hypertension or prehypertension — in them the effect can be noticeable after as little as two weeks. People with normal blood pressure and no cardiovascular risk factors also benefit (it's a generally healthy eating pattern), but the effect is less dramatic than in a population with elevated blood pressure. A real challenge tends to be consistently limiting sodium — this requires careful label reading and cutting back on processed food, the main source of sodium in a typical diet.

History of use

The DASH diet emerged directly from a trial funded by the U.S. National Institutes of Health (NIH), published in 1997 by a team led by Larry Appel. A follow-up trial, DASH-Sodium (2001), combined the DASH pattern with three levels of sodium restriction, becoming one of the first studies to precisely measure the dose-response relationship between sodium and blood pressure. Since then, DASH has been regularly recommended by American and European cardiology societies as a first, non-pharmacological step in treating hypertension.

Mechanism of action

A high intake of potassium, magnesium, and calcium from vegetables, fruit, and low-fat dairy improves the sodium-potassium balance in the kidneys, limiting sodium and water retention. At the same time, reduced sodium intake (a target of 2,300 mg, or 1,500 mg daily in the DASH-Sodium variant) directly lowers circulating blood volume and peripheral vascular resistance.

The high fiber and polyphenol content from vegetables and fruit, combined with limited saturated fat, further improves vascular endothelial function — it increases nitric oxide bioavailability, which promotes blood vessel relaxation. The effect is largely additive: the eating pattern alone lowers blood pressure even without changing sodium intake, but combining both elements (as in the DASH-Sodium trial) produces the strongest, dose-dependent drop.
