Overview
A lipid panel is a blood test panel that includes total cholesterol, LDL cholesterol ('bad'), HDL cholesterol ('good'), and triglycerides, usually done after fasting. It's one of the most commonly ordered preventive tests, and at the same time one of the more frequently misread — the total cholesterol number alone, without breaking it down into fractions, says relatively little about actual cardiovascular risk.
The 2019 American College of Cardiology / American Heart Association guidelines emphasize that it's LDL cholesterol specifically, and especially its long-term cumulative exposure, that is the main, causally linked risk factor for atherosclerosis and cardiovascular disease — the lower the LDL and the longer it's kept low, the lower the risk, with no clearly identified lower safety threshold.
Who can this realistically help? Every adult, as part of routine cardiovascular prevention, and especially people with a family history of heart disease, excess weight, diabetes, or other risk factors. Interpreting a single result without context (age, other risk factors, family history) can lead to unnecessary worry or, conversely, false reassurance.
Mechanism of action
LDL cholesterol transports cholesterol from the liver to peripheral tissues; excess circulating LDL, especially its smaller, denser particles, can penetrate the walls of blood vessels, where it becomes oxidized and triggers an inflammatory cascade that leads to the formation of atherosclerotic plaques. This process unfolds gradually over decades, which explains why cumulative, long-term LDL exposure matters more than any single measurement.
HDL cholesterol serves a partly opposite function — it participates in reverse cholesterol transport, moving cholesterol from peripheral tissues back to the liver, where it can be excreted from the body, which partly explains why it's called 'good' cholesterol, though the relationship is more complex than that simplification suggests. Triglycerides, meanwhile, mainly reflect current energy balance and insulin sensitivity — elevated levels often co-occur with insulin resistance and metabolic syndrome.
