Pregnancy Nutrition: What to Eat and What to Avoid

"Eating for two" is one of the most misleading phrases in pregnancy nutrition — actual calorie needs rise far less than the saying implies, while the need for specific nutrients rises considerably. Folate, iron, iodine, choline, and DHA have documented effects on a baby's development, and a handful of specific food categories — raw fish, unpasteurized cheese, high-mercury fish, alcohol — carry real, well-documented risk. "Eating for two" is a myth — nutrient density matters, not calorie count

The popular belief that a pregnant woman should "eat for two" leads in practice to excessive weight gain with no real benefit to the baby. Actual additional calorie needs are far more modest than the phrase suggests: in the first trimester they barely increase at all, in the second trimester they rise by about 340 kcal a day, and in the third by about 450 kcal a day relative to pre-pregnancy needs — the equivalent of one sensible extra snack, not doubled portions at every meal.

Far more important than calorie count is the nutrient density of what's eaten during pregnancy. The need for a handful of specific nutrients — folate, iron, iodine, choline, and omega-3 fatty acids — rises in pregnancy considerably more than overall calorie needs, meaning the diet has to deliver more of these nutrients while total food intake grows only modestly. It's these specific deficiencies, not raw calorie count, that carry documented risk for pregnancy outcomes and fetal development.

Folate — the best-documented element of a pregnancy diet

Of all pregnancy nutrition recommendations, the one concerning folate (vitamin B9) rests on the strongest clinical evidence. Folate is essential for DNA synthesis and cell division, which makes it especially critical in the earliest weeks of pregnancy, when the fetal neural tube — the structure that becomes the brain and spinal cord — is forming.

This randomized trial enrolled 1,817 women at elevated risk of a neural-tube-defect-affected pregnancy (due to a prior such pregnancy), assigned to receive folic acid, other vitamins, both, or neither. Women taking folic acid had a 71% lower risk of a neural tube defect in their child compared with women not taking it. The results were so clear-cut that the trial was stopped early for ethical reasons.

Based on this and later data, ACOG recommends that most women of reproductive age take at least 400 micrograms of folic acid daily — ideally starting before conception, since the neural tube closes by around day 28 of pregnancy, often before a woman even knows she's pregnant. Women at elevated risk, for example after a prior neural-tube-defect-affected pregnancy, are advised to take a much higher dose, around 4 milligrams daily, but only under a doctor's supervision — that's a therapeutic dose, not the standard for every pregnant woman.

Iron — rising demand and the risk of anemia

Circulating blood volume increases by as much as 40-50% during pregnancy, dramatically raising the need for iron to produce the additional hemoglobin required. The recommended daily iron intake in pregnancy is about 27 mg — substantially more than the standard 18 mg recommended for an adult woman outside pregnancy. Uncompensated deficiency leads to iron-deficiency anemia, linked in studies to a higher risk of preterm birth, low birth weight, and pronounced maternal fatigue that's easily mistaken for "normal" pregnancy tiredness.

In practice, most standard prenatal vitamins contain iron close to the recommended dose, but with confirmed anemia on bloodwork, a doctor may prescribe additional, higher-dose supplementation.

Iodine and choline — less talked about, but just as important for the baby's brain

Iodine is essential for producing thyroid hormones, which in turn drive proper fetal brain development, especially in the first half of pregnancy, before the baby's own thyroid becomes functional. Iodine needs rise in pregnancy to roughly 220-250 micrograms daily. Even moderate maternal iodine deficiency has been linked in population studies to lower IQ in children, making this micronutrient genuinely underrated compared with folate or iron, despite comparable importance for neurological development.

Women in their third trimester were randomly assigned to consume either 480 mg or 930 mg of choline daily until delivery. Children of mothers in the higher-dose group showed significantly faster information processing speed on cognitive tests at 4, 7, 10, and 13 months of age. The study was small (24 mother-child pairs with complete data) but randomized and diet-controlled, which strengthens confidence in the result despite the small sample.

The standard recommended choline intake in pregnancy is about 450 mg daily, though Caudill et al.'s study suggests higher doses (900+ mg) may bring additional cognitive benefits for the baby — that's still a single, small study, though, not grounds for substantially exceeding standard recommendations on your own without consulting a doctor. Choline is found mainly in egg yolks, liver, and meat, which puts people on plant-based diets at particular risk of deficiency and warrants considering supplementation after consulting a doctor or dietitian.

DHA — the omega-3 fatty acid critical for brain and vision

Docosahexaenoic acid (DHA) is a long-chain omega-3 fatty acid intensively accumulated in the fetal brain and retina, especially in the third trimester. Typical recommendations call for at least 200-300 mg of DHA daily during pregnancy and breastfeeding — easiest to reach with 1-2 servings a week of low-mercury fatty fish (e.g., salmon, sardines) or, with lower fish intake, a DHA supplement from algae or fish oil. Fish and mercury — how to choose safely instead of avoiding fish altogether

Fish is one of the best sources of DHA, but some species, especially large predators at the top of the food chain, accumulate methylmercury in their tissue — a neurotoxic compound particularly dangerous to a developing fetal nervous system. Rather than recommending avoiding fish altogether, the FDA and EPA publish detailed joint guidance on which species are safe and which to avoid.

Moderate evidence indicates that fish intake in pregnancy supports the baby's cognitive development, so avoiding fish entirely "just in case" deprives the diet of a valuable DHA source without a real safety benefit, as long as low-mercury species are chosen.

A practical rule: meat and fish are best eaten well cooked or reheated to a high temperature, and cheese labels should be checked for "pasteurized."

Alcohol and caffeine — why "a little" isn't the same as "safe"

Myth

An occasional glass of wine or non-alcoholic beer during pregnancy won't hurt, since the dose is so small.

Fact

No safe dose of alcohol or safe time to drink it during pregnancy has been established — that's why the CDC, ACOG, and virtually every major medical body recommend complete abstinence throughout pregnancy. Alcohol crosses the placenta freely, and its use is linked to the risk of fetal alcohol spectrum disorders (FASD), which involve varying degrees of physical, cognitive, and behavioral developmental impairment.

In practice, the 200 mg caffeine limit covers not just coffee but tea, energy drinks, cola, and chocolate too — it's worth adding up caffeine from all these sources across the day rather than counting only cups of coffee. Signs not to simply "wait out" on your own

Severe, persistent vomiting that prevents keeping down fluids and food (possible hyperemesis gravidarum, requiring hospital treatment), sudden significant weight gain or swelling accompanied by headache and vision changes (possible signs of preeclampsia), severe, worsening fatigue with shortness of breath that could indicate advanced anemia, and unusual intense thirst with frequent urination (possible signs of gestational diabetes) are all signals warranting urgent contact with the physician managing the pregnancy — not self-directed changes to diet or supplementation while waiting for the next routine appointment.
