Restless legs syndrome — common, persistent, and still underdiagnosed

Restless legs syndrome (RLS) is a neurological disorder that patients describe in a strikingly consistent way: an unpleasant sensation in the legs that's hard to pin down precisely — tingling, pulling, "crawling," sometimes outright pain — combined with an almost irresistible urge to move the legs. The key clinical feature that separates RLS from ordinary muscle fatigue is its clear time pattern: symptoms worsen at rest, especially in the evening and at night, and at least partly ease with movement. It's exactly this dependence on time of day and stillness that makes RLS strike precisely where it hurts most — at the moment of falling asleep.

For many people with RLS, the evening turns into a repeating ritual: getting into bed, mounting discomfort in the legs, getting up and pacing the room, brief relief, another attempt at falling asleep, and the same cycle repeating, sometimes for an hour or longer. The result is prolonged sleep onset, frequent awakenings, and chronic sleep deprivation, which over time translates into daytime fatigue, difficulty concentrating, and a worsened mood — and because the disorder's underlying mechanism is often invisible on standard tests, many patients spend years being told it's "stress" or "restless sleep" before anyone asks directly about symptoms matching RLS.

RLS is, at the same time, a relatively common disorder in the general population, and its severity varies widely — from mild, occasional discomfort to a form severe enough to practically prevent normal, uninterrupted sleep on many nights of the week. It's precisely this variability in severity, and the frequent mistaking of symptoms for other causes of insomnia, that make the topic of RLS treatment — including the role of iron — worth a separate, careful discussion instead of being lumped into a general bucket of "sleep problems."

Iron in the brain, not just in the blood — where this connection comes from

The link between iron and restless legs syndrome is one of the better-established areas of research into this disorder, but it has an important feature that's easy to overlook: it largely concerns iron in specific brain structures — above all the substantia nigra, an area involved in regulating movement via dopamine — not just iron circulating in the blood. Imaging studies and post-mortem analyses of brain tissue in people with RLS have repeatedly shown reduced iron content in these brain regions, even in patients whose standard blood tests didn't reveal clear, severe anemia.

This distinction — central (brain) iron versus peripheral (blood) iron — explains why some RLS patients have a perfectly reasonable-looking blood count and yet still have severe symptoms: the body's iron stores can be low enough that functions critical for survival (red blood cell production) get priority, at the expense of the nervous tissue's less "urgent" needs from the body's point of view. The exact mechanism by which reduced iron in these brain regions translates into RLS's movement symptoms is still being studied and shouldn't be presented here as fully explained — but the direction of the association (lower central iron, more frequent and severe symptoms) has been repeated in the RLS literature for years.

From a practical standpoint, the most important consequence of this link is that in RLS patients, doctors pay attention not so much to whether the patient has overt anemia, but to the ferritin level — the iron-storage protein we discuss in more detail in our entry on iron — even when it still technically falls within the laboratory "normal" range. It's exactly this low-normal, rather than overtly deficient, ferritin range that became the subject of the study discussed later in this article.

What the study in patients with low-normal ferritin showed

A randomized, double-blind, placebo-controlled trial lasting 12 weeks, involving patients with diagnosed RLS and ferritin in the low-normal range (not necessarily overt iron deficiency or anemia). Baseline symptom severity, measured with the IRLS scale (International Restless Legs Scale), was similar in both groups (treatment group: 24.8±5.72 points; placebo group: 23.0±5.03 points). After 12 weeks, symptom severity decreased significantly more in the group taking oral iron (a drop of 10.3±7.40 points) than in the placebo group (a drop of 1.14±5.64 points; p=0.01). Ferritin levels also rose more markedly in the treatment group (+25.1±20.3 ng/mL) than in the placebo group (+7.5±13.7 ng/mL; p=0.04).

In other words: both groups started with similarly severe RLS symptoms, but after 12 weeks the group taking oral iron saw a significantly greater reduction in symptoms than the placebo group, and the difference between groups was statistically significant. Although this is a single, relatively small trial, its strength lies in its design: randomization, double-blinding, and a placebo control group make it possible to attribute the observed improvement to iron itself, rather than to placebo effect or natural variability in RLS symptoms over time, with much greater confidence.

A study in a specifically selected population, not a general RLS group

A key methodological detail we return to in the next section: participants were included in the study precisely because their ferritin fell in the low, low-normal range. This wasn't a trial in a random group of people with RLS regardless of iron status — and this participant selection is fundamental to who these results actually apply to.

Why patient selection is the single most important detail of this entire study

The participants in this study weren't recruited at random from among everyone with restless legs syndrome — they were deliberately selected because their ferritin fell in the low, low-normal range. That means the study's results speak above all to the effectiveness of iron in RLS patients with exactly that iron profile, confirmed by a blood test — not to whether iron would help someone with RLS and a normal, high ferritin level, whose symptoms likely have an entirely different underlying cause (for example genetic factors, other medications, or neurological or kidney disease). Extrapolating this result to anyone with restless legs, regardless of their iron status, would be overinterpreting the data.

This detail has direct, practical significance for anyone reading about this study hoping for a quick fix to their RLS problem: the diagnosis "I have restless legs syndrome" alone isn't enough to know whether iron supplementation even has a chance of working. A specific blood test is needed — a ferritin level, ideally together with a full blood count and, if the doctor considers it warranted, additional iron-status markers — before anyone starts treating RLS symptoms with iron supplementation hoping it will work the way it did in this study.

It's also worth remembering that the "low-normal" ferritin discussed in this study and in the broader RLS literature is a completely different reference point than the lower limit of normal used to diagnose classic iron-deficiency anemia. The thresholds used in RLS research tend to be higher than the diagnostic thresholds for anemia — so a ferritin result that rules out anemia doesn't necessarily rule out an iron status that matters for RLS. This is one more reason to leave the interpretation of a specific result to a doctor who knows the patient's full clinical context, rather than comparing the number against a general laboratory reference range on your own.

Ferritin and iron diagnostics — the broader context

This diagnostic limitation applies to RLS patients too — if a ferritin result is ambiguous, or an inflammatory condition or infection is present at the same time, it's worth having the doctor take that into account when interpreting it, rather than basing a decision on a single number alone.

It's also worth emphasizing that iron supplementation itself — including when it's motivated by RLS symptoms — follows the same general rules we describe in our entry on iron: the form of the preparation, the timing relative to meals, and the gap from coffee, tea, or calcium all have a real effect on how much iron actually gets absorbed. Starting supplementation without accounting for these factors can mean a much weaker effect than the one observed under the controlled conditions of a clinical trial.

Yet it's precisely the ferritin test result — not the mere fact of having RLS symptoms — that determines whether a given patient even fits the population in which this intervention showed effectiveness in a clinical trial.

Why it's not worth supplementing iron "just in case"

Iron is not a neutral supplement to "try out"

Unlike many supplements, whose excess the body simply excretes, iron has no active mechanism for removing surplus — it can gradually accumulate in the liver and other organs with prolonged, unnecessary supplementation in someone who doesn't actually have low iron levels. This is one reason why the decision to start iron supplementation for RLS symptoms — like for any other reason — is always worth preceding with a blood test and medical consultation, rather than making it on your own based on symptoms alone.

Before reaching for iron because of RLS symptoms

  • Get a ferritin test done (ideally together with a full blood count) — this is the essential starting point, without which it's hard to assess whether supplementation even has a chance of working
  • Discuss the result with a doctor, taking the clinical context into account — ferritin is an acute-phase protein, and a single result can be difficult to interpret conclusively without a fuller picture
  • Don't assume upfront that RLS symptoms mean iron deficiency — there are many other possible causes and aggravating factors for RLS that a doctor should consider
