The most common cause of anemia worldwide — and one of the most often overlooked

Iron-deficiency anemia is a condition in which the body lacks the iron needed to produce hemoglobin — the protein in red blood cells responsible for transporting oxygen to tissues. It is by far the most common cause of anemia worldwide, considerably more frequent in women than in men due to blood loss during menstruation and increased iron demand during pregnancy. The problem with this diagnosis isn't a lack of available tests — a complete blood count and ferritin are cheap, widely available, and routinely ordered. The problem is that the symptoms of iron-deficiency anemia are general and nonspecific enough that patients (and sometimes doctors) easily attribute them to fatigue, stress, or "just feeling run down," rather than considering a specific blood test as a first step.

An analysis of data from 204 countries and territories within the Global Burden of Disease Study 2021 found that dietary iron deficiency remains the leading cause of anemia, accounting for 66.2% of all anemia cases worldwide. In 2021, this affected roughly 825 million women and 444 million men combined across all causes of anemia, with a markedly greater burden on women and girls than on men and boys. Notably, from 1990 to 2021 the decline in anemia prevalence was much larger in men (about 24%) than in women (about 7%), indicating slower progress on this problem in the female population despite decades of public health programs.

These numbers show more than just the scale of the problem — they also show that despite widespread availability of diagnosis and treatment (oral iron supplements are cheap and sold over the counter in many countries), progress in reducing the anemia burden in women is clearly slower than in men. This suggests the issue isn't purely one of treatment access, but also of recognizing symptoms and referring patients for the right tests.

How iron deficiency is actually diagnosed — the criteria

Diagnosing anemia relies on two main parameters: hemoglobin concentration (indicating anemia itself) and ferritin concentration (indicating the cause — the body's iron stores). Under commonly used criteria, anemia is diagnosed at hemoglobin below 13 g/dL in men and below 12 g/dL in non-pregnant women, though exact thresholds may vary slightly between guidelines.

Laboratory diagnosis of iron-deficiency anemia: an overview

A classic systematic review of diagnostic studies established the accuracy of serum ferritin as a marker of iron deficiency at various cutoff thresholds. At a threshold below 45 ng/mL, sensitivity for detecting iron deficiency was 85%, and specificity was 92% — meaning the test correctly identifies most people with a true deficiency, with a relatively low false-positive rate. At the more restrictive threshold below 15 ng/mL, sensitivity dropped to 59%, but specificity rose to 99%. This work is what established ferritin as the primary diagnostic marker for iron deficiency in clinical practice, while also cautioning that choosing a cutoff threshold is a trade-off between catching more cases and the number of false diagnoses.

Ferritin is an acute-phase protein — inflammation can distort the result

Ferritin also rises in inflammation, infections, and certain chronic diseases, independent of iron stores. In someone with active inflammation, a seemingly normal ferritin result may mask a true iron deficiency — which is why doctors sometimes pair ferritin with CRP or other markers to interpret the result correctly.

Symptoms that are easy to overlook or attribute to something else

The most common symptoms of iron-deficiency anemia — chronic fatigue and breathlessness on exertion — result directly from the disease mechanism: less hemoglobin means less oxygen reaching tissues, including muscles and the brain. The problem is that the same symptoms accompany dozens of other, far more mundane causes — sleep deprivation, chronic stress, hypothyroidism, or simply overwork.

Myth

Iron-deficiency anemia mainly shows up as pale skin and visible weakness — if I don't look "pale," I probably don't have it.

Fact

Pale skin is indeed a classic sign, but it often appears only with more severe deficiency and can be hard to judge yourself, especially in people with darker skin tones, where paleness of the conjunctiva or mucous membranes is a better indicator. Earlier and more frequent symptoms include fatigue disproportionate to exertion, breathlessness climbing stairs, headaches, heart palpitations, difficulty concentrating, and — less often recognized — brittle nails, hair loss, and pica: a compulsive craving to eat non-food substances (e.g., ice, known as pagophagia).

Symptoms worth associating with possible iron deficiency

  • Chronic fatigue disproportionate to sleep and exertion levels
  • Breathlessness on exertion that previously wasn't a problem
  • Paleness of the skin, conjunctiva, or oral mucous membranes
  • Frequent headaches and dizziness, heart palpitations
  • Brittle nails, in advanced cases spoon-shaped nails (koilonychia)
  • Hair loss, inflammation of the tongue (glossitis)
  • Pica — a compulsive craving for non-food substances, including pagophagia (craving to chew ice)

Fatigue can appear before full-blown anemia develops

An important, often overlooked detail: iron deficiency can cause symptoms — fatigue above all — even before hemoglobin drops enough to meet formal anemia criteria. This is known as iron deficiency without anemia (low iron stores, normal hemoglobin), a state that's often missed by standard diagnostics focused solely on the blood count.

Iron supplementation for unexplained fatigue in non-anaemic women: double blind randomised placebo controlled trial

Moderate evidence

144 women with unexplained chronic fatigue but no anemia (normal hemoglobin) were randomized to oral iron (80 mg daily) or placebo for 4 weeks. Fatigue decreased by 1.82 points on a 10-point scale in the iron group versus 0.85 points in the placebo group — a difference of 0.95 points (95% CI 0.32-1.62; p=0.004). The effect was limited to women with ferritin at or below 50 µg/L at baseline — women with higher ferritin gained no additional benefit from supplementation over placebo.

Moderate evidence

This result shows that symptoms linked to low iron can occur and partly reverse with supplementation even before full anemia develops — but this trial involved women with normal hemoglobin, not people with already-diagnosed iron-deficiency anemia, the main subject of this article. It shouldn't be treated as evidence for self-supplementing iron for fatigue without first checking ferritin via a blood test — excess iron also carries health risks.

What not to do on your own

Iron supplementation shouldn't be started without first confirming a deficiency via blood testing — excess iron in the body (hemochromatosis, iron overload) is a harmful condition that's harder to treat than deficiency itself, and its symptoms can be just as nonspecific as those of deficiency. Ferritin as a single result can be distorted by concurrent inflammation, so interpreting borderline results should always take into account the full clinical picture assessed by a doctor, not just a number on a lab printout.
