Overview

Ferritin is a protein that stores iron inside cells, mainly in the liver, spleen and bone marrow. A small amount of it enters the bloodstream in proportion to the size of the body's iron stores, which makes serum ferritin the best single, widely available indicator of the body's iron status — far more sensitive than the blood iron concentration itself, which fluctuates strongly over the course of a day.

Interpreting ferritin can be tricky, however, because it's also an acute-phase protein — its level rises in response to inflammation, infection or liver disease, regardless of actual iron stores. This means low ferritin almost always confirms iron deficiency, but normal or elevated ferritin doesn't always rule out deficiency if concurrent inflammation is masking genuinely low stores.

Who can this realistically help? People with symptoms suggesting iron deficiency (fatigue, paleness, hair loss, exertional breathlessness), women with heavy periods, blood donors, vegetarians and vegans, and endurance athletes, whose iron requirements can be elevated.

Mechanism of action

Ferritin stores iron in a non-toxic form, protecting cells from the oxidative damage that free iron could cause. When iron stores are low, cells release it from ferritin stores for transport (via transferrin) to where it's needed, primarily to the bone marrow for hemoglobin production — this process gradually depletes storage reserves, which is reflected as a drop in blood ferritin concentration.

At the same time, ferritin is regulated by pro-inflammatory cytokines (including interleukin-6) independently of iron stores — during infection or chronic inflammation, the body increases ferritin production partly as a defense mechanism (limiting the availability of free iron to pathogens), which can artificially inflate the result despite a coexisting iron deficiency.
