"Histamine intolerance" has become one of the most frequently searched terms in recent years, alongside lactose or gluten intolerance. The symptoms attributed to it are remarkably broad — headaches, skin flushing, hives, diarrhea, heart palpitations, a runny nose, even anxiety — which itself is worth noting: a list this nonspecific easily becomes a catch-all for various unexplained complaints.

The biochemical mechanism behind it is, however, real and well described. Histamine is a naturally occurring biogenic amine that acts as both a neurotransmitter and a mediator of inflammatory and allergic reactions, but it's also present in many foods — especially fermented, aged, and long-stored ones. In most people, dietary histamine is efficiently broken down in the gut by an enzyme called diamine oxidase (DAO) before it can trigger systemic effects.

This isn't the same as a food allergy

Histamine intolerance isn't an IgE-mediated reaction like a classic food allergy — it can't be detected with standard allergy tests. It's a metabolic phenomenon tied to impaired histamine breakdown, not an immune system reaction to a specific allergen.

The mechanism: when the DAO enzyme can't keep up

Diamine oxidase is produced mainly by the epithelial cells of the small intestine, and that's where the first line of dietary histamine breakdown happens, before the substance can enter the bloodstream. When DAO activity is reduced — regardless of the cause — more dietary histamine gets absorbed, and its concentration in blood and tissues rises above the threshold at which symptoms like vasodilation, itching, rapid heartbeat, or altered gut motility appear.

The risk of an overly restrictive elimination diet

A low-histamine diet followed independently and without a time limit can be very restrictive — it eliminates entire food groups, including some vegetables, citrus fruits, nuts, fermented foods, and alcohol. Followed long-term without a dietitian's guidance, it can lead to deficiencies in protein, calcium, B vitamins, and fiber, and in some people reinforces anxious, restrictive eating patterns. It should be treated as a temporary diagnostic tool, not a lifelong way of eating.

  • Freshness matters — histamine levels rise with storage time and the fermentation or aging process, so the same fresh meat and the same aged meat can differ substantially in histamine content

  • Consult a clinical dietitian about the elimination, especially if symptoms are severe or the diet is meant to last longer than a few weeks

Limitations of the current evidence

Most available studies on the low-histamine diet and DAO supplementation have small sample sizes (20-30 people), short observation periods, and, in some cases, no control group. No large, multicenter randomized trial has yet confirmed the efficacy of these interventions in the general population with suspected histamine intolerance — the available data mainly concern a narrow group of patients with chronic urticaria, not everyone reporting nonspecific symptoms after eating.
