The Wim Hof method rests on three pillars: a specific breathing technique involving controlled hyperventilation, gradual cold exposure (from cold showers to ice baths), and a mental component — focus, determination, and accepting discomfort. The method owes its popularity to Wim Hof himself, known for his cold-endurance records, but what sets it apart from a mere curiosity is that one of its components — the breathing technique combined with cold exposure — was tested in a controlled scientific study published in a prestigious journal.

This guide focuses mainly on the breathing technique itself — how to perform it step by step, what the study that inspired it actually showed, and, just as important, what precautions are needed before anyone tries it on their own.

The breathing technique, step by step

A basic breathing round

  • Sit or lie down in a comfortable, safe position — never in water, in a bathtub, or behind the wheel
  • Take 30-40 deep, fairly rapid inhales with passive exhales through the mouth or nose, without a full exhale each time — this is the controlled hyperventilation phase
  • After the last inhale, let the air out and hold your breath for as long as is comfortable (breath retention)
  • When you feel a strong urge to breathe, take one deep breath and hold it for about 15 seconds, then release it
  • Repeat the full cycle 3-4 times per session, with normal breathing in between rounds

Tingling and lightheadedness are a normal response to hyperventilation

Tingling in the hands, mild lightheadedness, or a sense of euphoria during the hyperventilation phase result from a drop in blood carbon dioxide levels (hypocapnia) and are a described effect of this technique, not a sign something has gone wrong. However, this exact mechanism is also behind the most important risk described further below.

The study tested whether a ten-day training program combining meditation, a breathing technique, and cold exposure could allow conscious influence over typically involuntary autonomic and immune responses. A trained group (12 people) and an untrained control group were both given an intravenous bacterial endotoxin that induces a transient, flu-like inflammatory response. The trained group showed significantly higher epinephrine (adrenaline) levels and a faster rise in the anti-inflammatory cytokine IL-10 compared with controls. At the same time, levels of the pro-inflammatory cytokines TNF-α, IL-6, and IL-8 were lower in the trained group, who also reported milder flu-like symptoms after receiving the endotoxin.

This study was groundbreaking because it had generally been assumed that the response of the autonomic (sympathetic) nervous system and the innate immune system to an inflammatory stimulus was largely involuntary and not subject to conscious control. A result suggesting that a short training program combining breath, cold, and mental elements could modify that response opened a new line of research into potential applications for inflammatory and autoimmune conditions.

What this study does NOT prove

A small sample and three interventions combined at once

The study included just 12 people in the trained group — a very small sample given the conclusions marketing materials sometimes try to draw from it. Just as important, participants underwent a full ten-day program combining the breathing technique, meditation, and cold exposure simultaneously — the study cannot clearly isolate how much of the observed effect is attributable to the breathing training alone versus the rest of the program. Treating the breathing technique in isolation as fully scientifically validated based on this one result would be an overinterpretation.

Myth

Since the study showed changes in cytokines, the Wim Hof method cures autoimmune diseases and boosts resistance to infections.

Fact

The study tested an acute response to an experimentally administered endotoxin in healthy volunteers, not the long-term course of autoimmune disease or susceptibility to real-world infections. The authors themselves described this as a promising, preliminary direction for further clinical research in specific conditions, not a proven, ready-to-use treatment or preventive method.

Safety — the most important thing before you start

Never in water, never behind the wheel

Controlled hyperventilation lowers blood carbon dioxide levels and can delay the body's natural breathlessness signal that warns against oxygen deprivation, which — with too long a breath hold — can lead to loss of consciousness. This is especially dangerous in water — drownings have been documented among people practicing hyperventilation techniques before or during breath-hold diving. For this same reason, the breathing technique should only be performed sitting or lying down in a safe place, never in a bathtub, pool, in water, or while driving or operating machinery.

Who should avoid this technique or consult a doctor first

  • People with epilepsy or a history of seizures — hyperventilation is a known trigger for seizures in some patients
  • People with cardiovascular disease, including uncontrolled hypertension or arrhythmia
  • Pregnant women
  • People with anxiety disorders or a history of panic attacks — the intense physical sensations during hyperventilation can worsen anxiety symptoms
  • People with asthma or chronic obstructive pulmonary disease — worth discussing the technique with a treating doctor

How to start safely, if you still want to try it

Cautious first steps

  • Start with fewer breaths in your first round (e.g., 15-20) rather than the standard 30-40, to gauge your own response
  • Always practice sitting or lying down, in a safe, familiar setting, ideally with someone nearby for your first attempts
  • Never perform the technique right before or during any activity in water
  • Introduce cold exposure gradually and separately from learning the breathing technique itself — e.g., start with a colder finish to your shower, well after mastering the breathing basics
  • Stop the exercise if you experience a strong headache, heart palpitations beyond mild acceleration, or significant anxiety

It's worth keeping it in proper proportion, though: it's a solid basis for further research, not a fully validated therapy. The breathing technique itself, practiced safely and with care, can be a valuable tool for working with your response to stress and discomfort — but it shouldn't replace medical treatment or be used without regard for the contraindications described here.

The most interesting takeaway from this study isn't "breathing cures disease" — it's "the line between what's involuntary and what can be consciously trained is less rigid than we thought," which is a very different, far more cautious statement.
