Your watch shows your HRV, but the measurement itself changes nothing. HRV training (biofeedback) is a specific technique — breathing at a resonance rate, usually around 6 breaths per minute — with real scientific support for reducing stress and anxiety. Here's how it works and how to start.

HRV as a metric vs. HRV as training — two different things

In our knowledge-base entry on heart rate variability, we describe HRV as a passive metric — something you measure in the morning to gauge your recovery state. But there's an entirely different application of the same phenomenon: HRV training (HRV biofeedback), an active technique where you deliberately modulate your breathing rate to temporarily increase heart rate variability and — as studies suggest — lower perceived stress and anxiety.

This distinction matters, because on social media the two uses get constantly conflated. Just measuring HRV on a watch does nothing therapeutic — it's only a number. HRV training is a separate, active intervention with its own distinct evidence base.

A meta-analysis of 24 studies, including both randomized trials and studies without a control group, found a statistically significant reduction in self-reported stress and anxiety symptoms after HRV biofeedback training. The Goessl et al. meta-analysis is the strongest available evidence for the stress-reduction effect, but it largely rests on smaller, methodologically varied studies — exactly the problem the Lalanza et al. review describes. That doesn't invalidate the effect, but it does mean the size of the effect in your individual case may differ from the meta-analysis's averaged result.

What resonance frequency is and why it matters

At the heart of this technique is the phenomenon of cardiovascular resonance: breathing at a specific rate, individual to each person — usually around 4.5–7 breaths per minute, far slower than the resting 12–18 — causes the heart-rate oscillations tied to breathing to overlap with the naturally slower oscillations in blood pressure (the baroreceptor reflex). The result is a maximal, temporary increase in HRV amplitude.

Resonance frequency is individual — it varies between people depending on height and cardiovascular structure, which is why precise research protocols start by determining it (usually by testing several breathing rates and checking which one produces the largest HRV amplitude) before the actual training begins. In home practice, a good starting point is a rate of around 6 breaths per minute (5 seconds in, 5 seconds out) — a value close to resonance for most adults.

The mechanism isn't fully mapped out yet

While the cardiorespiratory resonance phenomenon is well documented physiologically, the exact mechanism by which regular training translates into long-term reduction of anxiety and stress is still under investigation — hypotheses include strengthened vagal nerve activity, improved baroreceptor sensitivity, and central-nervous-system effects tied to emotion regulation.

What a typical protocol looks like

A basic, at-home HRV training protocol

  • Find a quiet spot, sit comfortably with a straight back
  • Breathe at a rate of around 6 breaths per minute: 5 seconds inhale through the nose, 5 seconds exhale through the mouth or nose
  • Focus on a smooth transition between inhale and exhale, without holding your breath
  • Start with 5–10 minutes a day, gradually extending to 15–20 minutes
  • Apps and some fitness watches offer a visual or vibration pacer for breathing rate, which makes it easier to hold the rhythm

Clinical protocols vary in length — from a few weeks to several months of regular practice, usually several times a week. The Lalanza et al. review notes there isn't yet one universally confirmed "optimal" dosage — more practice generally correlates with a larger effect, but the exact dose-response curve isn't fully established.

What HRV training doesn't do

Myth

HRV training is a quick way to "reset" your nervous system in a few minutes and permanently raise your baseline HRV.

Fact

Available evidence primarily concerns self-reported stress and anxiety after regular, multi-week practice — there's no solid evidence that a single session permanently raises your baseline, morning-measured HRV. The short-term effect (during the exercise itself) and the long-term effect (a shift in your baseline trend) are two different things that are easy to conflate.

It's also worth remembering that reduced stress and anxiety in study populations doesn't automatically mean the technique will work identically for you — as with most psychophysiological interventions, individual response tends to vary.

People with severe respiratory or cardiovascular conditions should consult a doctor before starting regular breathing practice.
