Overview
Melatonin is a hormone released by the pineal gland (a small gland in the brain) in response to darkness, signaling to the body that sleep time is approaching. Its secretion rises in the evening, peaks in the middle of the night, and falls off toward morning — a process strongly suppressed by light, especially blue light, which is why evening screen exposure can delay natural sleep onset.
Melatonin works most strongly and predictably in people with circadian rhythm disorders — after crossing multiple time zones, during shift work, or with delayed sleep phase (falling asleep and waking up much later than the people around you). In these groups, supplementation resynchronizes the body clock rather than just 'knocking you out' for a while. People with classic primary insomnia — driven by stress rather than a circadian disorder, for example — tend to see a noticeably more modest effect, because melatonin doesn't act as a general sedative, only as a chronobiological signal. It's also worth remembering that more isn't better: in studies, doses of 0.5–1 mg have proven just as effective as 5–10 mg, so reaching for a high dose 'to be safe' usually doesn't add any extra sleep-inducing effect — it mainly raises the risk of next-day grogginess.
It's worth stressing that melatonin isn't a classic 'sleeping pill' in the sense of benzodiazepines or Z-drugs — it doesn't cause direct sedation, but acts as a chronobiological signal that synchronizes the body clock. That's why supplementation is particularly well studied and effective for jet lag, shift work, and delayed sleep phase, and less so as a universal remedy for every kind of insomnia.
History of use
Melatonin was first isolated and described in 1958, when the American dermatologist Aaron B. Lerner studied extracts from cattle pineal glands while searching for a substance that lightens amphibian skin — hence the name, which references its effect on melanocytes. Only in the following decades was its key role as a hormone synchronizing the human circadian rhythm uncovered, which led to its commercial use as a supplement supporting sleep onset and coping with jet lag.
Mechanism of action
Melatonin binds to MT1 and MT2 receptors in the suprachiasmatic nucleus of the hypothalamus (SCN) — the body's master 'clock' that coordinates the circadian rhythms of all tissues — synchronizing the circadian rhythm with the light-dark cycle. The MT1 receptor is mainly linked to suppressing wakefulness, while MT2 is linked to shifting the phase of the body clock, which explains melatonin's dual use: as immediate support for falling asleep and as a way to resynchronize rhythm when crossing time zones.
The signal that triggers melatonin production itself runs in the opposite direction: light detected by specialized retinal ganglion cells (containing melanopsin) suppresses signaling along the retinohypothalamic tract leading to the SCN, and from there — via the sympathetic nervous system — to the pineal gland, where the enzyme AANAT (aralkylamine N-acetyltransferase) controls the rate at which melatonin is synthesized from serotonin. MT1 and MT2 receptors are Gi protein-coupled receptors, so their activation lowers cAMP levels in SCN neurons and dampens their electrical activity — this is the direct mechanism by which melatonin 'quiets' the body clock and shifts its phase.
