Overview
The almond mushroom (Agaricus blazei Murill, classified after a 2005 taxonomic revision mainly as Agaricus subrufescens; also encountered under the names Agaricus brasiliensis or 'sun mushroom') is an edible mushroom originally described from Brazil, where it is traditionally called 'himematsutake' or 'sun mushroom' (cogumelo do sol). It reached Japan in the 1960s and became a popular immune-support dietary supplement ingredient there.
Unlike turkey tail, whose PSK/PSP polysaccharides are a registered oncology drug in Japan, the almond mushroom remains primarily a subject of basic research and small clinical studies with mixed, sometimes contradictory results. That makes its evidence level lower and less consistent than turkey tail's.
Who actually benefits from the almond mushroom, and who probably doesn't? The most promising, though still isolated, data concerns people with standardly treated type 2 diabetes, in whom adding the extract was associated with improvement in the HOMA-IR index — but that's one study, not a confirmed, repeatable relationship. People counting on immune support should be aware that another randomized study found no significant effect on inflammatory markers — the evidence here is clearly inconsistent, unlike, say, turkey tail. Because of isolated reports of hepatotoxicity with long-term use of high doses of unpurified preparations, it is also less suitable for people with liver disease.
Where it's found
Agaricus blazei originates from the Piedade region of São Paulo state in Brazil, where it grows naturally on grassland with exceptionally high sun exposure — hence the local name 'cogumelo do sol' (sun mushroom). It is now also cultivated commercially in Japan, China, and South Korea.
History of use
The mushroom was traditionally gathered by local people near Piedade in Brazil before it caught the attention of Japanese researchers and farmers in the 1960s, who brought it to Japan under the name 'himematsutake'. It quickly became popular there as an immune-support supplement, even though — unlike turkey tail — it has never been registered as a drug.
Mechanism of action
Like other medicinal mushrooms, the almond mushroom contains beta-glucans and other polysaccharides credited with immunomodulatory activity through stimulation of dendritic cells, macrophages, and lymphocytes, as well as an effect on cytokine secretion. It also contains ergosterol (a vitamin D2 precursor) and antioxidant phenolic compounds. Research on carbohydrate metabolism has suggested a possible effect on insulin sensitivity, though the mechanism has not been clearly established and some results have not been replicated in independent studies.
The immunomodulatory effect is mainly attributed to a branched beta-(1→6)-D-glucan fraction, which immune cells recognize via the Dectin-1 receptor and Toll-like receptors on macrophages and dendritic cells, leading to secretion of pro-inflammatory cytokines (including TNF-alpha, IL-6). Regarding the effect on carbohydrate metabolism, it has been hypothesized that Agaricus blazei polysaccharides may increase tissue insulin sensitivity through mechanisms related to AMPK activation — however, unlike beta-glucan-driven immunomodulation, this mechanism remains poorly studied directly for this species.

Frequently asked questions
Preliminary studies suggest a beneficial effect on some immune parameters in cancer patients and people with inflammatory bowel disease. However, other studies found no effect on inflammatory markers. The evidence is mixed.
A small randomized study in patients with type 2 diabetes treated with metformin showed improved insulin resistance after adding the extract. That's a promising but isolated result — it doesn't replace diabetes treatment.
Both mushrooms are studied for immune support, but turkey tail has considerably stronger clinical evidence — its PSK/PSP polysaccharides are a registered drug used alongside chemotherapy in Japan.

The species' taxonomic identity was disputed for decades — only in 2005 was it finally established that the Agaricus blazei sold in supplements is mostly Agaricus subrufescens.
In Brazil, the Piedade region the mushroom comes from has a microclimate with unusually high sun exposure, hence the local name 'cogumelo do sol' (sun mushroom).
