Maca Root, Libido, and Energy

Maca (Lepidium meyenii) is a Peruvian root from the cabbage family, eaten in the Andes for centuries as food and sold in the West for the past decade as a natural "aphrodisiac" and energy booster. A handful of small but genuinely conducted clinical trials actually do show an improvement in sexual desire with maca supplementation — and, interestingly, without any change in testosterone levels, one of the more curious and less well-known findings in this field. The second part of the marketing promise — "energy" — is far less documented, with evidence that is, in practice, almost entirely anecdotal.

An Andean Root That Became a Global Supplement

Maca (Lepidium meyenii) is a tuberous plant of the cabbage family (Brassicaceae — the same family as broccoli or turnip), cultivated for thousands of years on the high plateaus of the Peruvian Andes, at altitudes of 3,500–4,500 meters, where hardly any other cultivated plant survives at all. In the traditional diet of the Junín region, maca was primarily a food — the dried root was boiled, roasted, or fermented into a drink called chicha de maca — and the role attributed to it in supporting fertility and vitality has deep roots in local folk tradition.

In the West, maca reached the supplement market in the late 1990s and was quickly wrapped in marketing slogans linking it to libido, energy, and "natural Viagra" — a promise catchy enough that for years it overshadowed the question of which of these specific claims actually have real backing in clinical research, and which are simply a traditional, folk use translated into the modern language of supplement marketing.

Two Different Promises, Two Different Levels of Evidence

This article deliberately separates two claims that maca marketing often sells together: (1) the effect on sexual desire (libido) — where several real RCTs exist with a specific, repeatable result, and (2) the effect on "energy" in the sense of general arousal and reduced fatigue — where clinical evidence is considerably weaker and largely indirect.

What the Key Libido RCT Showed

A randomized, double-blind, placebo-controlled study in healthy men aged 21–56, who took gelatinized maca at a dose of 1,500 mg or 3,000 mg daily, or placebo, for 12 weeks. An improvement in sexual desire (assessed via a self-report scale) was observed starting from week 8 of supplementation and persisted through week 12. Logistic regression analysis showed the effect on libido was independent of mood changes (Hamilton depression and anxiety scales) and — particularly notably — independent of blood testosterone and estradiol levels, which did not differ significantly between the maca and placebo groups.

This result is worth highlighting, since it contradicts the most popular explanation circulating online about maca — that it works "hormonally," by raising testosterone. The study by Gonzales and colleagues, one of the few specifically designed to test this, showed the opposite: the effect on sexual desire appeared without any measurable change in testosterone or estradiol levels. This means that if maca genuinely does affect libido, it does so through a non-hormonal mechanism that has not yet been fully explained.

What the Systematic Review of All Available RCTs Shows

A systematic review searching 17 databases through April 2010 for all randomized clinical trials of maca versus placebo. Ultimately 4 RCTs qualified for analysis. Two of them showed a significant, positive effect of maca on sexual dysfunction or sexual desire — in healthy menopausal women and in healthy adult men, respectively. One study found no effect in healthy competitive cyclists. A fourth study, conducted in patients with erectile dysfunction using the IIEF-5 scale, showed significant improvement. The authors summarized the results as "limited evidence" for maca's effectiveness, noting that the number of studies, sample sizes, and average methodological quality of the primary studies were too small to draw firm conclusions.

Four Studies Aren't Enough for Certainty, but Enough for a Hypothesis Worth Attention

Early-stage evidence

The fact that a systematic review found only 4 qualifying RCTs on the entire topic of maca and sexual function itself says a lot about how early this field of research is. The direction of results is consistent in three of the four studies (improved desire or sexual function), but this is still a small, heterogeneous body of evidence — different populations (healthy men, menopausal women, athletes, ED patients), different doses and forms of maca, and different measurement tools make it difficult to generalize the result to everyone reaching for this supplement.

What About "Energy"? A Much Weaker Foundation Than Libido

Maca supplement marketing almost always combines two claims in one sentence: "improves libido and boosts energy." Yet these are two entirely different levels of evidence. While real, randomized studies with a specific numerical result exist for sexual desire, the clinical evidence for "energy" — in the sense of general arousal, reduced perceived fatigue, or improved physical performance — is far more modest, largely limited to sports-performance studies that failed to confirm the effect convincingly, plus traditional use and subjective accounts rather than dedicated, well-designed studies on fatigue.

A Study in Competitive Cyclists Found No Effect

In the study included in Shin et al.'s (2010) review, conducted in healthy, trained cyclists, maca supplementation showed no significant effect on performance outcomes or subjective feelings of energy compared with placebo — one of the few direct tests of the "energy" claim, and it happened to come back negative.

This doesn't mean maca definitely has no effect on perceived energy in anyone — but it does mean the "energy" claim today rests mainly on tradition, marketing, and isolated, unconfirmed observations, rather than on the same quality of evidence as the libido claim. It should honestly be labeled a hypothesis, not a confirmed effect.

Mechanism — Why the Effect on Libido Remains a Partial Mystery

Maca contains a group of compounds unique to this species called macamides and macaenes — fatty acid amides not naturally found in any other commonly consumed plant. Most of the described effects are attributed to them, though the exact mechanism by which they act on the central nervous system or sexual desire has not yet been fully explained at the molecular level in humans.

Since the Gonzales et al. study ruled out a hormonal mechanism (no changes in testosterone or estradiol), and the effect didn't result from a mood change either, what remains are hypotheses about a direct effect on brain structures involved in regulating sexual desire, or on vascular endothelial function and blood flow — similar mechanisms are sometimes studied in the context of erectile dysfunction. None of these hypotheses currently holds the status of a confirmed mechanism — this is an area of active preclinical research, mostly in animal models, which should not be confused with evidence of an effect in humans.

This Isn't the Same as Rhodiola or Ginseng

Unlike classic adaptogens such as rhodiola or ginseng, maca isn't typically classified as a plant that eases the body's stress response via the HPA axis — its studied effect profile centers almost exclusively on sexual function rather than general stress resilience or mental fatigue.

What These Studies Don't Prove

The evidence base for maca remains early-stage and small. The key libido RCT (Gonzales et al., 2002) included only a few dozen healthy men — a good study design doesn't fully compensate for a small sample. A significant portion of published maca research, including some cited in the Shin et al. review, comes from the same research team in Peru, raising questions about independent replication by other centers. Product standardization on the market is low — different studies used different doses (from about 1.5 g to 3 g daily) and different forms (gelatinized, raw, black, red), which makes direct comparison of results and translation to a specific store-bought product difficult. None of the available studies lasted longer than several weeks, so the effects and safety of long-term, multi-year use remain essentially unknown.
