Overview
Heart rate variability (HRV) is a measure of the fluctuation in time between successive heartbeats — counterintuitively, higher variability is a sign of a well-functioning, flexible autonomic nervous system, not irregularity or a problem. HRV reflects the balance between the sympathetic ('fight or flight') and parasympathetic ('rest and digest') branches of the autonomic nervous system.
HRV has gained enormous popularity in biohacking and sport thanks to its wide availability in sports watches and wearables, but the review by Shaffer and Ginsberg emphasizes that interpreting a single measurement without context can be misleading — HRV is strongly individual, depending on age, sex, training level and time of day, which is why the most valuable approach is tracking one's own trend over time, not comparing oneself to other people or general norms.

Mechanism of action
The heart is innervated by both sympathetic and parasympathetic (vagus nerve) fibers of the autonomic nervous system, which modulate in real time the intervals between successive beats in response to breathing, stress level and other physiological stimuli. A healthy, well-recovered body shows a high capacity for rapid, flexible modulation of this rhythm, which translates into higher variability — conversely, chronic stress, illness or insufficient recovery shift the balance toward sympathetic dominance, flattening this variability.
HRV measurement most often relies on analyzing R-R intervals from an ECG recording or photoplethysmography (PPG, used in watches), and the most popular time-domain metric is RMSSD, strongly correlated with vagus nerve activity. Because HRV changes dynamically in response to sleep, training, alcohol and psychological stress, its diagnostic value comes primarily from tracking the trend over time in the same person, not from a single, isolated reading.
