Zinc is often described as an almost universal wound-healing "accelerator" — from minor scrapes to venous leg ulcers that persist for months. The reality, as shown by the most rigorous systematic review available (Cochrane), is far more measured: in people with chronic leg ulcers, oral zinc supplementation, without first confirming an actual deficiency, did not statistically significantly speed up healing. Zinc as a "healing remedy" — where the popularity comes from

It's hard to find a guide on wound healing that doesn't mention zinc. Zinc supplements, zinc ointments, and zinc oxide dressings have been sold for decades as a way to speed up skin regeneration — from minor cuts to chronic, slow-healing ulcers in older adults, people with diabetes, or venous insufficiency. Zinc's popularity in this context didn't come out of nowhere — it has solid biochemical grounding. The problem arises when a convincing biological mechanism automatically translates, in popular belief, into certainty of a clinical effect in every patient, regardless of whether they actually have a deficiency of this element. That distinction — between "zinc is needed for healing" and "more zinc heals wounds faster" — is the core of this article.

Zinc's role in wound-healing biology

Zinc is a cofactor for more than three hundred enzymes in the body, and many of them are directly involved in healing: it participates in DNA synthesis and cell division needed to rebuild the epidermis, in collagen synthesis that builds new connective tissue, in the proper function of immune cells fighting wound infection, and in protein and cell membrane metabolism. In people with an overt zinc deficiency, wound healing is documented to be delayed, and the deficiency itself is sometimes a cause of chronic, recurring skin ulcers.

This mechanistic role — zinc as an essential building block and cofactor of the healing process — is the basis on which the recommendation to supplement zinc in patients with chronic wounds has long rested. But the clinical question is different from the biochemical one: does giving extra zinc to someone who doesn't necessarily have a deficiency speed up healing beyond what an already-adequate baseline level would provide.

What the Cochrane review showed

Oral zinc for arterial and venous leg ulcers

This Cochrane systematic review included six small randomized trials (183 participants total) comparing oral zinc sulphate with placebo or no treatment in people with venous, arterial, or mixed-etiology leg ulcers. Four trials involved venous ulcers, one arterial ulcers, one mixed ulcers. Pooling the four trials comparing oral zinc sulphate with placebo in venous ulcer patients showed no statistically significant difference in complete healing rates (risk ratio 1.22; 95% CI 0.88–1.68). The review author concluded there is no evidence that oral zinc increases the proportion of healed arterial or venous leg ulcers.

It's worth highlighting who the participants in these trials were: patients with chronic leg ulcers enrolled without prior selection based on a confirmed zinc deficiency. In other words, this tests the claim "extra zinc for every ulcer patient," not the claim "zinc for ulcer patients with a confirmed zinc deficiency." These two research questions, though superficially similar, lead to entirely different clinical conclusions.

Four of six trials measured serum zinc

Four of the six included trials measured participants' baseline zinc levels, which partly allowed assessment of whether the study populations actually had deficiencies of this element. The review author notes these populations varied in this regard, and each trial was too small to reliably carve out a subgroup with confirmed deficiency and evaluate the effect of supplementation in that subgroup alone.

Why the result is surprising despite a strong biological mechanism

At first glance, the lack of effect in clinical trials seems to contradict zinc's well-documented, crucial role in the cellular biology of healing. The resolution of this apparent contradiction is fairly intuitive: if the body already has enough zinc for all its zinc-dependent enzymatic processes to run properly, an extra dose of this element doesn't push them further — similar to adding more oil to an engine that already has the right oil level; it doesn't improve performance. The effect of supplementation shows up only when it removes an actual shortfall, not when it adds surplus to an already-sufficient state.

The weakness remains the size and quality of the source trials: six small trials (183 participants total), some with unclear risk of bias due to poor reporting. That's enough to fail to confirm an effect in an unselected population, but not enough to definitively rule out a benefit in a narrower, well-selected subgroup of patients with a genuine deficiency.

Deficiency versus normal levels — the key distinction

Who actually might have a zinc deficiency

Groups at higher risk of zinc deficiency where supplementation may make sense

  • Older adults — zinc absorption and dietary intake often decline with age, and this is precisely the group most affected by chronic leg ulcers
  • Patients with diabetes — metabolic disturbances and more frequent absorption problems can favor micronutrient deficiencies, including zinc
  • People with chronic gastrointestinal diseases (e.g., inflammatory bowel disease, prior bowel resection) that impair absorption
  • Malnourished patients or those with very low intake of animal protein — the main dietary source of absorbable zinc
  • Patients with a chronic wound whose healing remains unusually prolonged despite treating the underlying cause (e.g., successful compression therapy for a venous ulcer)
  • Patients with documented, substantial wound exudate loss — exudate from a chronic wound can itself contain significant amounts of zinc and accelerate its loss from the body

How to check zinc levels — and why it's not straightforward

In theory, the answer to this dilemma is simply testing blood zinc levels before deciding on supplementation. In practice, interpreting such a result is complicated: serum zinc concentration doesn't always faithfully reflect the body's total zinc stores, and zinc, like many other micronutrients, tends to be temporarily lowered during inflammation and infection — and a chronic wound is itself just such an inflammatory state. This means a blood test result in a patient with an ulcer might suggest deficiency that's actually a result of the inflammatory state itself, rather than genuinely low zinc reserves in the body.

For this reason, interpreting the result and deciding on supplementation is best left to a doctor who will take into account the patient's full clinical picture — diet, comorbidities, the course of the wound itself — rather than basing it solely on a single, borderline lab result.

Why it's not worth supplementing zinc "just in case"

Excess zinc is not neutral

Long-term supplementation with high doses of zinc without a confirmed deficiency can disrupt copper absorption, leading to a secondary copper deficiency and related hematological and neurological complications. Excess zinc can also cause gastrointestinal complaints and weaken immune response — the opposite of the intended effect in a patient with a chronic wound susceptible to infection.

This is another argument for basing the decision to supplement zinc in the context of chronic wound healing on a specific clinical indication, rather than on the assumption that "more can't hurt." With this particular element, it can hurt — unlike many water-soluble vitamins, which the body simply excretes in excess.
