This distinction matters in practice, because the two strategies differ not just in schedule but in the nature of the challenge they pose — 16:8 requires daily discipline within a shorter window, while 5:2 requires a considerably stronger calorie restriction, but only on two days a week. There were also no significant differences in insulin, fasting glucose, or other metabolic markers between groups, and estimated energy intake didn't differ significantly between them either.

In other words: in this particular, well-designed trial, simply restricting the eating window without any accompanying calorie counting didn't prove significantly more effective than a traditional meal schedule for weight loss. The authors clearly concluded that eating within a restricted time window, on its own, without additional interventions, is not more effective than eating spread across the whole day.

This doesn't mean 16:8 'doesn't work' for everyone

The result of this trial mainly concerns the mechanism of restricting the eating window itself, without deliberate calorie counting. For many people in practice, a 16:8 window works indirectly — it simply makes it easier to maintain a calorie deficit, for example by eliminating evening snacking — rather than because the timing of meals itself has a magical effect on metabolism independent of overall energy balance.

What a trial comparing alternate-day fasting with a reducing diet showed

The 5:2 method wasn't tested in its exact classic form in this particular trial, but a very similar, cyclical protocol — alternate-day fasting, in which calorie intake is restricted to about 25% of typical needs every other day — was compared directly against a traditional, daily reducing diet in a year-long clinical trial.
