Hyaluronic Acid for Joint Pain: What the Evidence Shows for Injections and Oral Supplements

Hyaluronic acid is mostly associated today with facial cosmetics, but in medicine it's also been injected directly into joints for decades — to relieve pain in knee osteoarthritis — and, less commonly, taken as an oral supplement. These are two very different interventions with very different strength of evidence, often conflated in popular articles. We check what a large meta-analysis comparing injections to other treatments actually showed, why the American orthopedic association advises against routine injections, and what happens when hyaluronic acid is simply swallowed instead.

Hyaluronic acid's natural role in the joint

Hyaluronic acid is a natural component of synovial fluid — the thick, viscous fluid filling the joint cavity that reduces friction between cartilage surfaces and acts as a shock absorber during movement. In osteoarthritis, both the concentration and the molecular weight of hyaluronic acid in synovial fluid decrease, which theoretically weakens its protective and lubricating properties — this observation is exactly what led to the idea of replenishing it directly in the joint through so-called viscosupplementation.

It's worth distinguishing right away between two very different interventions often treated as the same thing in casual conversation: intra-articular hyaluronic acid injections, administered by a doctor directly into the joint cavity, and oral hyaluronic acid supplements, taken as tablets or capsules. The mechanism of action, the route the substance takes to reach the joint, and — as it turns out — the quality of the evidence are fundamentally different in each case.

Intra-articular injections: what a large meta-analysis showed

The most influential summary of evidence in this area is a network meta-analysis published in Annals of Internal Medicine, comparing nearly every available pharmacological treatment for knee osteoarthritis — from acetaminophen, through nonsteroidal anti-inflammatory drugs, to intra-articular hyaluronic acid and corticosteroid injections.

This network meta-analysis covered 137 randomized trials (33,243 participants) comparing various pharmacological treatments for knee osteoarthritis. Intra-articular hyaluronic acid showed the strongest pain-relief effect versus oral placebo among all interventions assessed, with an effect size of 0.63 (95% credible interval 0.39–0.88). All interventions outperformed oral placebo, with acetaminophen showing the smallest effect (0.18), and intra-articular treatments generally outperforming NSAIDs. The authors noted that the advantage of intra-articular methods may partly stem from a so-called integrated placebo effect tied to the act of injection itself, rather than the active substance alone.

Strong evidence

The fact that intra-articular hyaluronic acid scored highest among 137 compared studies sounds like an unambiguous recommendation — but the meta-analysis's own authors explicitly noted the effect may be partly a methodological artifact. The comparison was against oral placebo (a pill), not a saline injection into the joint — the act of injecting anything into a joint, regardless of the substance, produces a stronger placebo effect than swallowing a pill. This distinction, easy to miss in simplified summaries, is key to interpreting the result correctly.

Why orthopedic associations advise against routine injections

Despite its high score in the network meta-analysis, the American Academy of Orthopaedic Surgeons (AAOS), in its 2013 clinical practice guideline, issued a strong recommendation against the routine use of hyaluronic acid injections for knee osteoarthritis, citing improvement that failed to clear the threshold for clinically important difference in a portion of high-quality trials. In the updated 2021 guideline, that recommendation was softened to moderate strength, but the direction stayed the same — injections are not recommended for routine use.

Conflicting guidelines in clinical practice

It's worth knowing that not all medical societies share the AAOS position — some more recent guidelines (including part of OARSI's 2019 recommendations and the 2020 VA/DoD guidelines used in the US veterans' healthcare system) permit hyaluronic acid as an option for select patients, especially those who haven't responded to other conservative treatments. This divergence between guidelines reflects exactly the ambiguity and methodological caveats described in the Bannuru et al. meta-analysis — it isn't a communication error, but a reflection of a genuine dispute within the expert community.

What about hyaluronic acid in pill form?

Oral hyaluronic acid supplements are marketed as a more convenient, non-invasive alternative to injections. Biologically, though, this is a completely different intervention: hyaluronic acid taken orally has to pass through the digestive tract, be absorbed, and somehow reach the joint — a far less direct process than injecting the substance straight into the joint cavity, and for that reason harder to justify mechanistically from the outset.

This small pilot trial enrolled 20 people with knee osteoarthritis, taking 80 mg of oral hyaluronic acid daily or placebo for 8 weeks. The primary pain outcome measure (WOMAC) was similar at baseline in both groups (6.6 points in the active group versus 6.4 in placebo) and improved significantly in both groups over the trial, with no statistically significant difference between them. The hyaluronic acid group showed numerically larger improvement in physical function (-13.1 vs. -10.1) and total symptoms (-18.6 vs. -15.8), but these differences did not reach statistical significance.

Early-stage evidence

This trial matters precisely because it shows the opposite of the popular marketing narrative: on the primary, pre-specified pain measure, oral hyaluronic acid was not significantly better than placebo. Small, numerical differences favoring the active group on secondary measures can easily be due to chance in such a small sample (20 people) and don't constitute a solid basis for claiming effectiveness. This is the kind of result that's easy to miss if you only read a summary that highlights the numerical differences without noting the lack of statistical significance on the primary endpoint.

What this data doesn't prove

Limitations worth keeping in mind

The Bannuru et al. meta-analysis focused mainly on the knee — extrapolating the results to other joints (hip, shoulder, temporomandibular) is limited and shouldn't be treated as equivalent. The meta-analysis's own authors noted insufficient long-term data, incomplete safety reporting, and possible publication bias. The oral hyaluronic acid trial had a very small sample (20 people) and a short observation period (8 weeks), which prevents drawing strong conclusions — larger, longer trials are needed before this form of supplementation can be clearly evaluated. None of the studies discussed here was conducted in a population with very advanced osteoarthritis already qualifying for joint replacement, where effectiveness of both forms tends to be even more limited.
