Fibromyalgia has long been described as a disease that "doesn't show up" on any standard blood test or scan — which for decades led some doctors to dismiss patients' symptoms, and patients on a frustrating journey between specialists in search of a diagnosis. Today we know it is a real, well-described disorder of pain processing in the central nervous system, not a figment of imagination or "nerves." There's no single pill that switches it off — but there is a solid set of interventions with real research support that, in practice, help most patients regain function, even if they don't eliminate pain entirely.

A disease that long went unrecognized as a disease

Fibromyalgia is a chronic syndrome of widespread musculoskeletal pain, accompanied by fatigue that doesn't respond to rest, unrefreshing sleep, and cognitive disturbances — colloquially known as "fibro fog." For decades, the problem wasn't the existence of these symptoms themselves, but the lack of a clear marker on standard tests: normal blood counts, ESR, CRP, and joint imaging in a patient reporting pain in nearly every part of the body led some doctors to conclude that the problem was "in the patient's head" rather than in their nervous system.

This distinction has clinical, not just semantic, significance. Fibromyalgia is now classified as nociplastic pain — a third, distinct category alongside nociceptive pain (resulting from tissue damage) and neuropathic pain (resulting from nerve damage). In nociplastic pain, the pain-processing system itself, in the spinal cord and brain, becomes hypersensitive, amplifying signals that in a healthy person would be filtered out as irrelevant. This isn't imagined pain — it's genuinely altered physiology of stimulus processing, which can be partly measured and partly modified, though no single intervention reverses it completely.

This article doesn't promise a single solution — because, honestly, science doesn't have one yet. Instead, we go through what meta-analyses and clinical trials actually show about exercise, drug therapy, and the mechanism of the disease, so you can tell interventions with real, if moderate, evidentiary support apart from marketing promises with nothing behind them.

How fibromyalgia is even diagnosed

The current 2016 American College of Rheumatology (ACR) criteria, a revision of the 2010/2011 criteria, base diagnosis on two indices: the Widespread Pain Index (WPI — the number of painful body areas on a 0–19 scale) and the Symptom Severity Scale (SSS — assessing fatigue, unrefreshing sleep, cognitive symptoms, and somatic symptoms, on a 0–12 scale). Diagnosis is made with WPI ≥7 and SSS ≥5, or WPI 4–6 and SSS ≥9, with symptoms needing to persist at a similar level for at least 3 months, and pain needing to involve at least 4 of 5 defined body regions.

2016 Revisions to the 2010/2011 fibromyalgia diagnostic criteria

A revision of fibromyalgia diagnostic criteria that introduced, among other things, a requirement for widespread pain in at least 4 of 5 body regions (intended to exclude diagnoses based solely on regional pain) and allowed diagnosis regardless of coexisting rheumatic disease. These criteria are now the clinical and research standard, replacing the earlier approach based mainly on the number of tender points assessed by palpation.

An important change from older criteria

The criteria from the 1990s relied almost exclusively on the count of 18 defined "tender points" assessed by pressure — an approach criticized for high inter-examiner variability and for overlooking fatigue or cognitive disturbances as a legitimate part of the clinical picture. Current criteria no longer require a tender-point exam, which makes diagnosis more accessible in primary care, but also makes it more dependent on a thorough patient history.

What happens in the nervous system — the central sensitization mechanism

Central sensitization is a state in which the central nervous system amplifies the processing of sensory stimuli across multiple systems at once — with neuronal plasticity that increases sensitivity to future stimulation. In practice, this manifests as hyperalgesia (an exaggerated pain response to a stimulus that would normally hurt less), allodynia (feeling pain from stimuli that normally don't hurt at all, e.g., light touch), and a weakening of the natural, descending pain-inhibiting mechanisms.

This review points to three overlapping mechanisms: central sensitization with elevated levels of excitatory neurotransmitters (substance P, glutamate) and reduced serotonin and norepinephrine; peripheral sensitization with heightened nociceptor response; and small fiber neuropathy, present in roughly half of fibromyalgia patients. The authors also emphasize the involvement of inflammatory and immune mechanisms acting in parallel with the neurological changes.

This multifactorial picture explains why fibromyalgia rarely responds well to a single intervention targeting only one mechanism — for instance, a peripherally acting painkiller alone, without any effect on central pain processing, often brings limited relief. It also explains why approaches combining several strategies at once (movement, sleep, symptom-targeted drug therapy) have better research support than any one of them alone.

Exercise — the best-documented non-pharmacological intervention

A meta-analysis of 14 RCTs (715 patients, 700 of them women) compared different types of exercise. Strength training produced the largest effect on pain (SMD -1.39) and symptom severity (SMD -0.84) and significantly improved physical and mental quality of life. Aerobic exercise also significantly reduced pain (SMD -1.05) and symptom severity (SMD -0.65). Stretching alone did not significantly reduce pain (SMD -0.94, not statistically significant), though it improved physical quality of life the most. Overall, exercise (all types combined) produced a strong effect on pain (SMD -1.11), global well-being (SMD -0.67), and depressive symptoms (SMD -0.40).

The practical takeaway from this data is fairly concrete: strength and aerobic training have the strongest evidentiary support for reducing pain, while stretching alone — though pleasant and good for quality of life — isn't enough on its own if the goal is a real reduction in pain. This matters because many fibromyalgia patients, fearing that pain will worsen after exertion, intuitively avoid strength training in favor of gentler stretching — the data suggest this approach, though understandable, may not deliver the maximum possible benefit.

Start very slowly

In some fibromyalgia patients, training that's too intense or introduced too quickly can temporarily worsen pain and fatigue — a phenomenon sometimes called "post-exertional malaise" in this context. A practical strategy, supported by research on exercise dosing, is to start at very low intensity and volume, gradually increasing the load at a pace the patient tolerates, ideally under the guidance of a physiotherapist familiar with the syndrome.

Drug therapy — three first-choice medications, and none of them is universal

Comparison of Amitriptyline and US Food and Drug Administration–Approved Treatments for Fibromyalgia: A Systematic Review and Network Meta-analysis

A network meta-analysis of 36 RCTs covering 11,930 patients (94.4% women) compared amitriptyline (used off-label) with FDA-approved fibromyalgia treatments. Amitriptyline showed higher effectiveness in improving sleep (SMD -0.97), fatigue (SMD -0.64), and quality of life (SMD -0.80) than approved medications. Duloxetine at 120 mg was more effective at reducing pain (SMD -0.33) and depressive symptoms (SMD -0.25). Pregabalin at 450 mg gave a similar effect on pain (SMD -0.30), and at 600 mg significantly improved sleep (SMD -0.60).

The practical conclusion from this network meta-analysis is that it makes sense to match the drug choice to the patient's dominant symptom, rather than treating any single medication as universally "best." A patient whose main problem is insomnia and exhaustion may benefit more from amitriptyline, while a patient with dominant pain and coexisting depression may respond better to duloxetine. This reflects the multifactorial nature of the disease itself described in the previous section — there's no single neurotransmitter responsible for all the symptoms, so there's no single drug that acts optimally on all of them at once.

Modest, but real effects — not a miracle cure

Moderate evidence

It's worth having realistic expectations about the size of the effect. Earlier Cochrane reviews estimate that duloxetine, milnacipran, and pregabalin provide significant pain relief (at least a 50% reduction) in roughly 1 in 10 adults with moderate to severe fibromyalgia pain over 4–12 weeks, with no evidence of sustained effectiveness beyond 6 months in most patients. This doesn't mean the drugs don't work — it means they work in a minority of patients to a degree the patient would call meaningful improvement, while for the rest the effect is partial.

Sleep and stress — not an add-on to treatment, but part of it

Unrefreshing sleep is one of the diagnostic criteria for fibromyalgia for a reason — sleep disturbances and chronic pain feed each other in a vicious cycle. Fragmentation of deep sleep (NREM stages) lowers the next day's pain threshold, and heightened pain makes it harder to fall and stay asleep the following night. Who is at higher risk of fibromyalgia

  • Women — a strong female predominance, with a female-to-male ratio estimated at around 4:1
  • People with other chronic pain syndromes or rheumatic diseases, e.g., rheumatoid arthritis or lupus — fibromyalgia can coexist independently of another disease
  • People with a history of chronic, uncontrolled psychological stress or trauma, though the cause-and-effect relationship here isn't one-directional or fully understood
  • People with long-standing sleep disturbances, regardless of the original cause of those disturbances
  • Middle age — most often diagnosed between ages 30 and 60, though it can occur at any age, including in teenagers
  • Family history — first-degree relatives of people with fibromyalgia have an elevated risk, suggesting a genetic component alongside environmental factors

What's better to avoid or treat with caution

Opioids are not a recommended treatment for fibromyalgia

Clinical guidelines consistently advise against the routine use of opioids in treating fibromyalgia — evidence of their effectiveness in this specific syndrome is weak, and the risk of developing tolerance, dependence, and paradoxically heightened pain sensitivity (opioid-induced hyperalgesia) is real with long-term use. If a patient is receiving opioids for fibromyalgia, it's worth treating that as a cue to discuss alternative, better-studied options with their doctor.

Not every pain calls for the same intervention

New, atypical pain or a significant change in the character of existing symptoms in someone with diagnosed fibromyalgia shouldn't automatically be attributed to the disease itself — it may indicate a separate, coexisting health problem requiring its own diagnostic work-up. Fibromyalgia doesn't "protect" against other diseases or explain away every new symptom.

What's worth doing in practice

Limitations of this data

What these studies don't prove

The exercise meta-analysis (Sosa-Reina et al.) covered a relatively small total number of patients (715) and studies with differing protocols, which limits the precision of the estimated effect sizes. The drug therapy network meta-analysis (Farag et al.) compares drugs indirectly, through a shared reference point (placebo), rather than always in trials that directly pit them against each other — a methodologically acceptable approach, but less certain than direct head-to-head comparisons. None of these studies identifies a single method as the solution for most patients — the consistent conclusion across the literature is that fibromyalgia requires a multicomponent, individually tailored approach, and response to treatment varies widely between patients.
