Why would the brain even need creatine

The brain, despite making up only about 2% of body mass, consumes a disproportionately large share of energy — and phosphocreatine, just as in muscle, serves neurons as a rapidly available energy buffer supporting ATP production at moments of increased demand. In theory, higher creatine availability in brain tissue could therefore support neuron function under conditions of increased cognitive load.

A stronger effect under sleep deprivation and metabolic stress

Interestingly, some of the studies included in the review found a more pronounced effect of creatine on cognitive function in sleep-deprived people or under acute metabolic stress than in well-rested people — suggesting creatine may work mainly as a buffer under conditions of increased brain energy demand, rather than as a universal intelligence "booster."

Creatine and vegetarians: a stronger baseline effect

The classic 2003 study by Rae and colleagues, one of the first to examine creatine and cognition, found a more pronounced improvement in working memory and processing speed in vegetarians than in meat-eaters — which makes biological sense, since a vegetarian diet supplies far less creatine from food, meaning baseline stores in the body (including likely in the brain) tend to be lower.

What about older adults?

Beyond young adults and vegetarians, older adults are the third group in which creatine's effect on cognition has been studied — a particularly relevant population given the natural decline in both muscle mass and certain cognitive functions with age. Available, though still limited, research suggests older adults may derive a comparable or even somewhat greater cognitive benefit from creatine supplementation than younger people, which partly aligns with the general hypothesis that creatine's effect on the brain is stronger where baseline phosphocreatine stores in tissue are lower — a condition that applies to both vegetarians and older adults.

A comparison of typical creatine supplementation protocols

It's worth stressing that research on creatine and cognition is far less extensive and less methodologically consistent than decades of solid research on creatine and muscular performance — there is not yet a separate, well-established dosing protocol "for the brain" distinct from the standard sports dosing.

Limitations of this research worth keeping in mind

Still an early stage of research

The review's authors highlight the small sample sizes in most available studies, the varied cognitive tests used across studies, and short observation periods — this is an area that needs more large, long-term studies before recommendations as strong as those for muscular effects can be made.

Our editorial recommendation

Creatine remains one of the best-studied supplements for physical performance — a potential additional cognitive effect, while promising, still needs more solid evidence before it can be treated as a primary reason for supplementation rather than a pleasant side benefit.

If someone asked me purely about the cognitive effect, I'd wait for more large studies. But since creatine already has solid evidence for muscular performance with minimal risk, a potential bonus for the brain is an added argument, not the main one.
