Bruxism is a repetitive activity of the jaw muscles involving clenching, grinding the teeth, or bracing and thrusting the jaw, with no functional purpose such as eating or speaking. Stress and anxiety — the best-documented risk factor

Of all the risk factors for bruxism, the link with psychological stress and anxiety has the strongest and most consistent support in the research — for both the sleep and awake forms. The mechanism likely involves heightened sympathetic nervous system activity and hypothalamic-pituitary-adrenal axis activation in people under chronic psychological strain, which translates into increased jaw muscle tension both day and night. Awake bruxism can be a habit you don't notice

Clenching your teeth during the day — for example while focused on work, driving in traffic, or scrolling on your phone — is often automatic enough that many people aren't aware of it until someone points it out or jaw pain appears. A simple self-monitoring technique — consciously checking a few times a day whether your teeth are clenched and deliberately relaxing the jaw — is often the first, simplest step in managing awake bruxism.

Consequences: teeth, the jaw joint, and headaches

The most direct, visible consequence of long-term bruxism is pathological wear of the biting surfaces of the teeth, which in advanced cases leads to dentin hypersensitivity, enamel microfractures, and even the need for prosthetic treatment of damaged teeth. This is usually the first sign that prompts a dental visit — not pain, but noticeable, accelerated wear of the dentition.

A second, equally significant consequence is overload of the temporomandibular joint (TMJ) and the surrounding chewing muscles. Repeated, excessive clenching and grinding acts on this joint much like chronic overload does on other joints in the body, leading to pain radiating to the temple, ear, or neck, clicking when opening the mouth, and limited jaw mobility. A third, often overlooked consequence is tension-type headaches, especially around the temples and forehead, frequently appearing in the morning — right after a night of intense sleep bruxism. In patients with unexplained morning headaches, especially combined with jaw tension upon waking, bruxism is worth considering as a possible, easily overlooked cause.

How bruxism is actually diagnosed

In most cases, a dentist makes the diagnosis based on characteristic tooth wear patterns, hypertrophy of the chewing muscles (sometimes visible as an enlargement of the jaw angles), and a history — information from the patient about morning jaw tension or from a partner about audible grinding at night. This is enough to establish a diagnosis of "probable" bruxism in most clinical situations.

A definitive diagnosis of sleep bruxism, with a quantitative assessment of episode frequency and intensity, requires polysomnography with recording of jaw muscle activity (electromyography) — the same type of study we describe in the context of diagnosing sleep apnea. In clinical practice, such a full study is mainly performed when a coexisting sleep disorder (e.g., apnea) is suspected or when symptomatic treatment isn't working, rather than routinely for every patient with suspected bruxism.

  • Cutting back on caffeine, especially in the second half of the day, and evening alcohol

A visit to a dentist or doctor is warranted for: visible, progressive tooth wear or fractures, chronic pain in the jaw, temple, or ear, limited ability to open the mouth widely, frequent morning headaches coexisting with jaw tension, or suspicion that bruxism worsened after starting a new medication. Bruxism rarely has a single cause — more often it's the result of several overlapping factors: stress levels, medications taken, caffeine and alcohol intake, and sometimes a coexisting sleep disorder. That's exactly why an approach focused solely on protecting the teeth with a guard, while practically justified and worth implementing, rarely suffices on its own — lasting improvement usually comes from additionally addressing the underlying cause of the problem.

A night guard saves the teeth, but it doesn't answer the question of why someone is clenching their jaw every night in the first place. That second question is worth asking — sometimes the answer is simpler than it seems, and requires nothing more than a conversation with a doctor about medication dosage, or an evening coffee had a few hours too late.
