Nebido is the brand name for testosterone undecanoate — testosterone esterified with undecanoic acid, an eleven-carbon fatty acid, which makes it by far the most lipophilic and longest-acting testosterone ester used in routine clinical practice. That's a fundamental pharmacological difference from the far more common esters covered elsewhere on this site — enanthate and cypionate — which require injections every one to two weeks. After an initial loading phase, Nebido is typically given once every 10 to 14 weeks, meaning four to five injections a year instead of dozens. That's a radically different treatment rhythm, and for some patients it's the main reason to choose this product — but it also comes with a different course of treatment, a different blood concentration curve, and a requirement that the drug be administered only in a medical setting.

An actual course of Nebido therapy starts with a loading phase. Based on the description of the standard regimen for this product (1000mg in 4mL of castor oil) in the paper by Morales and colleagues, the first injection is given at week zero, the second already at week six — noticeably sooner than the eventual maintenance rhythm would suggest — and only from that point does the regimen move into a maintenance phase with injections every twelve weeks, individually fine-tuned within a window of ten to fourteen weeks depending on the testosterone concentration measured just before the next injection (the so-called trough level). That earlier second dose has one purpose: to build up a sufficient depot of drug in the muscle tissue before the intervals between injections stretch out, so that testosterone levels don't drop below the therapeutic range before the depot from the first dose is topped up.

Nebido's pharmacokinetic curve is noticeably flatter than that of the shorter esters, which produce a clear concentration peak in the first few days after injection followed by a gradual decline until the next dose. In an 84-week phase III trial by Wang and colleagues involving 130 hypogonadal men (baseline testosterone below 300 ng/dL), the trough testosterone concentration — measured right before the next dose — stayed within the normal adult male range of 300–1000 ng/dL (10.4–34.7 nmol/L) from the very first injection onward, and DHT, estradiol, and their ratios to testosterone remained stable once steady state was reached. It's worth flagging one important, potentially confusing detail here: this trial used a 750mg dose in 3mL — the formulation registered in the US under the name Aveed — rather than the 1000mg in 4mL that is the European standard described by Morales. So if a reader comes across the name Aveed elsewhere, it's worth knowing that it's the same active substance (testosterone undecanoate) at a different dose and volume, registered under a different regulatory system — not a different drug.

In practice, this means that starting Nebido therapy looks different from starting a short-ester regimen. Every injection is 4mL of an oily solution given as a deep intramuscular injection, usually into the gluteal muscle — a considerably larger volume than a typical enanthate or cypionate dose, which in itself calls for more technical precision when administering it. That observation requirement isn't a formality — it stems from a real, documented safety signal specific to oily, high-volume testosterone undecanoate preparations: pulmonary oil microembolism (POME) and, less often, anaphylactic reactions related to the oily vehicle's components. A review by Adam and colleagues, covering 48 publications including 29 studies and nearly 8,000 patients treated with testosterone undecanoate, identified a combined 88 episodes of POME or post-injection cough across eight of those studies, and one postmarketing review recorded 223 cases out of 3,107,652 injections given — a fraction of a percent, but not zero, and serious enough that the FDA in the United States mandated a REMS (Risk Evaluation and Mitigation Strategy) program for the Aveed product, requiring at least 30 minutes of in-office patient observation after every dose. A postmarketing analysis by Pastuszak and colleagues, covering 4.3 years of surveillance (March 2014 to June 2018), reviewed 90,092 doses distributed and 633 safety reports, of which 28 confirmed POME cases were identified — a yearly reporting rate under 0.1% per injection. The large majority of episodes resolved on their own: of the cases described in detail, 13 of 17 resolved within 30 minutes or less, and over 60% required no medical intervention at all. History of use

Oral testosterone undecanoate (under names such as Andriol or Restandol) has been used clinically since the 1970s, exploiting absorption via the intestinal lymphatic vessels to bypass extensive first-pass liver metabolism. The long-acting intramuscular form was developed and clinically tested much later — the key pharmacokinetic and safety studies, including those describing the standard 1000mg dosing schedule given every 10–14 weeks after a loading phase, date to the mid-2000s. The product was registered in the European Union under the name Nebido, and only later in the United States, under the name Aveed, at a different dose (750mg in 3mL) and with an additional FDA-mandated REMS program requiring patient observation after every injection — a regulatory response to accumulated data on the risk of pulmonary oil microembolism and anaphylaxis.

Mechanism of action

The testosterone molecule in Nebido is esterified with undecanoic acid — a saturated fatty acid with an eleven-carbon chain, considerably longer than that of enanthate (seven carbons) or cypionate (eight carbons, counting the ring). The longer and more lipophilic the ester chain, the more slowly the molecule dissolves and releases from the injection site — that's the basic principle behind testosterone ester pharmacology, and undecanoate, with the longest chain among the esters used clinically, is its most extreme example. The ester itself is dissolved in an oily vehicle — a mixture of castor oil and benzyl benzoate — which, once injected intramuscularly, forms a local depot in the tissue from which the active substance is released gradually over many weeks.

Releasing free, biologically active testosterone from this depot happens in two steps: ester molecules must first slowly diffuse out of the oily reservoir into the surrounding tissue and blood vessels, and then be cleaved by plasma and tissue esterases, which remove the undecanoate group and release free testosterone capable of binding the androgen receptor. Because both of these steps — diffusion out of the depot and enzymatic hydrolysis — are spread out over time rather than happening in a burst, the resulting concentration curve is much flatter than with enanthate or cypionate, which produce a clear peak within the first few days after injection and a clear decline toward the end of the dosing cycle. Released testosterone undergoes the same downstream conversions as with other esters — partial aromatization to estradiol and reduction by 5-alpha-reductase to dihydrotestosterone (DHT) — and once the therapy reaches steady state, the ratios of these metabolites to testosterone stay relatively constant between injections, unlike the fluctuations seen with shorter esters.

Pulmonary oil microembolism (POME) has a different mechanism from testosterone's typical side effects and stems directly from the physical properties of the oily, high-volume preparation itself, not from the hormone. During an intramuscular injection, there's a small risk that microscopic droplets of the oily vehicle enter a damaged vein instead of staying in the muscle tissue, after which they travel through the venous bloodstream to reach the small pulmonary vessels, triggering transient coughing, shortness of breath, chest tightness, or dizziness — symptoms that typically appear within minutes of the injection. This is a distinct phenomenon from the rarer anaphylactic reaction, in which the immune system reacts with hypersensitivity to the oily vehicle's own components. That's precisely why, even though both events are rare, the standard of care for administering testosterone undecanoate preparations calls for observing the patient directly after the injection, in a setting where prompt medical help is available should either event occur.
