Overview

Citicoline (CDP-choline, cytidine-5'-diphosphocholine) is a compound naturally present in every cell of the body as an intermediate product of the Kennedy pathway — the biochemical route for producing phosphatidylcholine, a key component of cell membranes, including neuron membranes. As a supplement it has been available for decades, initially studied mainly in the context of stroke rehabilitation, and today increasingly used to support cognitive function in healthy people.

A study by McGlade and colleagues, one of the more frequently cited in the context of healthy, young adults, showed improved psychomotor speed and attention after citicoline supplementation in healthy young men — which sets it apart from many nootropics studied almost exclusively in a clinical context (e.g. after stroke or in dementia).

Who can realistically benefit from this? People looking for mild support for concentration and reaction speed, for whom choline as a single dietary component may be insufficient (e.g. with low intake of eggs and organ meats, the main dietary sources of choline). The effect is usually subtle and mild, not comparable to the action of classic stimulants.

Mechanism of action

After ingestion, citicoline is broken down in the body into choline and cytidine, which, after absorption and crossing the blood-brain barrier, are resynthesized into citicoline inside nerve cells, where it serves as a substrate for producing phosphatidylcholine — the main phospholipid of neuron membranes. The increased availability of cell membrane building material may support the structural integrity of neurons, particularly relevant in the context of recovery after injury.

Additionally, the released choline is a precursor of acetylcholine, a neurotransmitter key to memory processes and attention, which constitutes a second, independent mechanism through which citicoline may support cognitive function. Cytidine, in turn, supports nucleic acid synthesis and may play a role in dopaminergic neurotransmission, though this mechanism is less well characterized than the effect on acetylcholine.
