Vitamin D and Psoriasis — Does Oral Supplementation Ease Symptoms?

Topical creams and ointments containing vitamin D analogues have long been standard, prescription treatment for psoriasis — that much is well established in dermatology. Far less clear is whether ordinary oral vitamin D supplementation — the same kind many people take in autumn and winter for immunity — has any effect on the course of the disease. A 2021 systematic review and meta-analysis of randomized trials tried to answer that question, and the result is more ambiguous than it might seem at first glance.

Psoriasis is not eczema — an important distinction to start with

Psoriasis and eczema (atopic dermatitis) are often treated as the same thing in popular perception — both present as red, itchy, flaking skin, and both follow a chronic course with periods of flare-ups and remission. In reality, these are two distinct diseases with different immunological underpinnings, different typical lesion appearance, and partly different treatment, which makes this distinction more than just a terminological curiosity.

Psoriasis is a chronic autoimmune-inflammatory disease in which the immune system — specifically overactive Th17 lymphocytes and their associated cytokines, mainly interleukin-17 and interleukin-23 — instructs skin cells to multiply several times faster than normal. Instead of the typical epidermal renewal cycle of about 28 days, in psoriasis it takes just 3-5 days, producing the characteristic, well-demarcated, thick, silvery-scaling plaques, most often on the elbows, knees, scalp, and lower back.

Myth

Psoriasis and eczema (atopic dermatitis) are essentially the same skin condition, just with different names.

Fact

These are two distinct disease entities. Eczema is more closely tied to the skin barrier and allergic-type reactions (elevated IgE, frequent co-occurrence with asthma and allergies), and its lesions are usually more diffuse, oozing, and intensely itchy. Psoriasis has an autoimmune basis rooted in the Th17/IL-17/IL-23 axis, and its lesions are typically well-demarcated, thick, and covered with silvery scale. Biologic treatments effective for one disease often don't work for the other — which itself confirms that the mechanisms are different.

Psoriasis affects about 2-3% of the world's population and, beyond the skin burden, carries an elevated risk of psoriatic arthritis, metabolic syndrome, and cardiovascular disease — so it's not merely a cosmetic issue, but a chronic systemic disease with a visible skin component.

Vitamin D in dermatology: a long-standing topic, but not in the form most people think of

The link between vitamin D and psoriasis isn't a new idea found on the internet — it's been well established in clinical dermatology for decades, just in a completely different form than a supplement in drops or a capsule. Topical vitamin D analogues, such as calcipotriol, have long been standard, prescription components of treatment for mild-to-moderate psoriasis, often combined with topical corticosteroids. They act locally on the skin, inhibiting excessive keratinocyte proliferation through the same VDR receptors we described in more detail in our article on vitamin D3.

The key distinction in this article

This article deals exclusively with oral vitamin D supplementation — that is, tablets, drops, and capsules taken systemically — not with topical vitamin D analogues applied directly to the skin as prescribed by a dermatologist. The efficacy of the latter in treating psoriasis is well established and is not the subject of this article or the study described below.

The question that genuinely remains open is a different one: given that topical vitamin D helps, does raising its level systemically — through ordinary oral supplementation, the same kind used preventively in autumn and winter — also translate into easing psoriasis symptoms? This question makes biological sense, because VDR receptors are found not only in the skin but also in the immune cells involved in the inflammatory process characteristic of psoriasis. But biological plausibility of a hypothesis is not the same as confirmation in clinical trials — and that's exactly what the systematic review described below tried to check.

What exactly the 2021 systematic review examined

Effectiveness of oral vitamin D supplementation in lessening disease severity among patients with psoriasis: a systematic review and meta-analysis of randomized controlled trials

A systematic review and meta-analysis of randomized controlled trials (RCTs) evaluated the effect of oral vitamin D supplementation on psoriasis severity. Of 5,018 records screened, 4 studies were included in the qualitative analysis, and 3 studies in the quantitative meta-analysis proper. The main outcome assessed was the change in PASI (Psoriasis Area and Severity Index, the standard scale for psoriasis severity) after 6 months of supplementation. The initial analysis showed a small, statistically significant improvement: the mean difference (MD) was -0.92 PASI points (95% CI: -1.72 to -0.11). After applying a more conservative statistical correction (the Hartung-Knapp adjustment, standardly used in meta-analyses with a small number of component studies), the same mean difference of -0.92 was no longer statistically significant (the 95% CI widened to -2.21 to 0.38). The authors explicitly conclude that a beneficial effect of oral vitamin D supplementation in psoriasis patients could not be definitively confirmed.

It's worth pausing on what this statistical correction actually means, because it's the crux of the whole result. Standard meta-analysis methods assume a certain number of component studies at which uncertainty estimation is reliable. With only three studies pooled, as in this case, the result is much more susceptible to the influence of a single study or to methodological differences between them. The Hartung-Knapp adjustment is precisely a way of estimating uncertainty more honestly and conservatively in such a situation — and it was this more cautious method that showed the effect could not be reliably distinguished from zero.

How to read this result honestly

Moderate evidence

This is not a study that "disproved" the hypothesis about vitamin D's role in psoriasis — it rather showed that current evidence from randomized trials is too scarce and too inconsistent to confirm a beneficial effect with sufficient statistical certainty. The direction of the result (a negative mean difference, i.e., improvement) was present in both versions of the analysis — only the certainty with which that direction could be considered a real effect, rather than chance, changed.

Why the result is "ambiguous" rather than simply "negative"

The difference between "the effect doesn't exist" and "the effect couldn't be confirmed with sufficient certainty" tends to blur in everyday language, but in research methodology it matters a great deal. Three studies in a meta-analysis is a small sample by evidence-based medicine standards — for comparison, conclusions about vitamin D's effect on respiratory infections, described in our article on vitamin D3, were based on data from more than 10,000 patients across dozens of trials. With only three studies, differences in vitamin D dosing, supplementation duration, participants' baseline 25(OH)D levels, or baseline psoriasis severity can easily shift the result one way or another.

The figure of -0.92 PASI points itself, even if it had turned out fully statistically significant, is also worth weighing against its clinical significance. The PASI scale typically ranges from 0 to 72, and a reduction of at least 75% from baseline (so-called PASI 75) is often used as a clinically meaningful improvement benchmark in trials of psoriasis drugs, including modern biologic therapies. A change of under one point, even in the best-case scenario from this meta-analysis, is a small effect compared to what's expected from drugs dedicated to psoriasis.

Limitations of this data

What this review does not prove

First, the pool of component studies was small — only 3 studies in the quantitative analysis — which itself limits the reliability of any conclusion, regardless of its direction. Second, these studies differed methodologically (dosing, duration, populations), which the review's authors themselves flag as a source of uncertainty. Third, the review covered only oral supplementation — it says nothing about topical vitamin D analogues, whose efficacy in treating psoriasis is established separately and far more solidly. Fourth, the studies assessed change in the PASI score rather than hard endpoints such as patient quality of life or long-term disease remission. Finally, the review did not separately analyze the subgroup of patients with baseline vitamin D deficiency — it's possible that in people with a genuine, confirmed 25(OH)D deficiency the effect of supplementation would be more pronounced than the averaged result across the whole studied population, but current data cannot settle this.

What this means for someone with psoriasis considering supplementation

Myth

Since topical vitamin D helps with psoriasis, taking vitamin D in tablet form should work even better because it acts on the whole body.

Fact

Topical vitamin D analogues act locally, at high concentrations directly at the site of the skin lesion, in a form optimized for effects on keratinocytes — this is a different delivery mechanism than raising overall 25(OH)D blood levels through an oral supplement. This systematic review shows that the effect of oral supplementation on the course of psoriasis, if it exists at all, is small and statistically uncertain — this assumption cannot simply be "carried over" from topical treatment to oral supplementation without separate evidence.

Practical takeaways for someone with psoriasis

  • The foundation of psoriasis treatment remains whatever a dermatologist prescribes — topical vitamin D analogues, topical corticosteroids, phototherapy, systemic or biologic drugs in more severe cases. No oral supplementation replaces this treatment
  • Checking your own 25(OH)D level with a blood test makes sense regardless of psoriasis — vitamin D deficiencies are common in Poland, as we describe in more detail in our article on vitamin D3, and correcting them makes sense for bone health and immunity, independent of any effect on the skin
  • If a blood test confirms an actual deficiency, correcting it with a dose matched to the result is reasonable on its own — but it shouldn't be treated as a psoriasis therapy with proven efficacy, since current data don't support that
  • Supplementing "just in case," at high doses, without a prior vitamin D level test, has no support from this review and carries a real, though rare at typical doses, risk of overdose with prolonged use of very high doses
