Vitamin B12 and Peripheral Neuropathy in Vegans and Vegetarians

A plant-based diet and an invisible B12 problem

Vitamin B12 (cobalamin) occurs naturally almost exclusively in animal-derived foods — meat, fish, eggs, and dairy. This means people following a vegetarian, and especially vegan, diet are structurally exposed to lower dietary intake of this vitamin, regardless of how well-balanced the rest of their diet is in other respects. B12 deficiency usually develops slowly, because the body stores reserves in the liver sufficient for several years of function even with zero dietary intake — which, paradoxically, makes the problem harder to catch early, since symptoms appear only once those reserves run out, sometimes years after switching to a plant-based diet. Among the most serious, potentially irreversible consequences of chronic B12 deficiency is damage to the myelin sheath of nerve fibers, leading to peripheral neuropathy.

Even though the link between B12 deficiency and neuropathy has long been known, relatively few studies have measured this problem using concrete, objective functional tests in people on plant-based diets — most existing knowledge was based on descriptions of advanced, clinically overt cases, rather than a systematic look at subclinical changes at an early stage.

A cross-sectional feasibility study included 38 healthy, able-bodied adults aged 19-40 who reported exclusive adherence to a vegetarian or vegan diet for at least 3 years. Peripheral neuropathy was assessed objectively using a balance force plate, a vibration sensitivity tester, and pegboard tests of hand dexterity. Serum vitamin B12 and folate were measured using standard radioimmunoassay techniques. Twenty-six percent of participants had deficient or marginal vitamin B12 status (below 221 pmol/L). Participants with adequate B12 status scored 10% higher on the Purdue Pegboard assembly test and 20% higher on the adjusted left-hand functional dexterity test compared with participants with marginal-to-deficient B12 status (p<0.05).

In other words: nearly a quarter of healthy, young adults on a long-term plant-based diet already had marginal or deficient B12 levels — and those with worse B12 status performed worse on objective hand dexterity tests, despite being young and reporting good overall physical fitness. This matters because it shows that subclinical, detectable functional changes can appear before a B12 deficiency becomes severe enough to produce clear, obvious clinical symptoms.

This is a cross-sectional feasibility study, not a large population study

  • Regularly supplement vitamin B12 or eat foods fortified with it — this is the only reliable way to meet requirements on a fully plant-based diet
  • Consider periodic blood testing of B12 levels, especially after several years on a plant-based diet without regular supplementation
  • Pay attention to early, subtle signs, such as minor difficulty with precise hand movements, tingling in the fingers, or worsening balance, and don't dismiss them as mere fatigue
  • For a borderline total B12 result, ask your doctor about an additional methylmalonic acid test, which better reflects actual cellular B12 status
  • Don't assume that the variety and quality of a plant-based diet in other respects automatically protects against B12 deficiency — this is a separate matter requiring deliberate action

Limitations of this evidence

Small sample, preliminary result

The study included only 38 participants, making it a pilot study rather than a large, definitive research project. The authors themselves classify it as a feasibility study, aimed at testing the measurement method itself rather than delivering final, generalizable conclusions. Nor was it an interventional trial assessing whether B12 supplementation fully reverses already-existing functional changes — the small pilot component of the study addressing supplementation was a limited feasibility test, not a full efficacy trial.
