A man a few months into TRT starts sleeping worse, wants sex less, feels more irritable than usual — and lands on a forum, where someone in a similar situation writes: 'that's 100% high estrogen, take anastrozole 0.5 mg every other day, worked for me'. Buying tablets through an online pharmacy without a prescription takes less time than booking a visit with the doctor running your therapy — and that's where the problem starts, because that whole path skips the most important step: checking whether estradiol is actually elevated, and whether it's really responsible for these symptoms.

Which symptoms really point to estradiol, and which get pinned on it unfairly

Not every unpleasant symptom on TRT is related to estrogen, even if internet intuition suggests otherwise. It's worth separating symptoms into two groups: those that, in the literature and clinical practice, are relatively specific to estradiol excess, and those most often blamed on it, while in reality usually having other, more common causes.

The first group — symptoms genuinely worth linking to estradiol — is headed by nipple tenderness or hypersensitivity (mastodynia). This is usually one of the earliest, relatively specific signals that something related to estrogenic stimulation is happening in breast tissue — it often appears before any visible changes develop. A second reliable signal is water retention: a feeling of being 'puffy', swelling of the ankles, fingers, or face, sometimes a faster-than-usual weight gain not attributable to fat or muscle tissue. In more advanced, chronically untreated cases, visible gynecomastia develops — real breast-gland growth, felt as a firmer, tender lump under the nipple, not just soft fat tissue. Myth

I have low libido and am constantly tired on TRT — it has to be high estrogen, I need to crash it.

Fact

Libido and fatigue are among the least specific symptoms in all of medicine — they have literally dozens of possible causes: sleep quality, stress level, the quality of the testosterone therapy itself, thyroid function, depression, relationship problems, and even too-low, not too-high, estradiol. A study by Tan et al. (2015) in a large group of men treated with testosterone injections found no higher rate of low-libido complaints among patients with elevated estradiol — in that study, low libido was reported slightly more often by patients with estradiol in the normal range than by those with excess. Linking these two very general symptoms exclusively to estrogen, without checking anything else, is one of the most common diagnostic mistakes patients make themselves.

High Estrogen in Men After Injectable Testosterone Therapy: The Low T Experience

A retrospective analysis of a large group of men treated with testosterone injections, in which elevated estradiol (≥42.6 pg/mL) occurred in about 20% of patients. Contrary to the common assumption, elevated estradiol wasn't associated with a higher rate of low-libido complaints — among patients reporting libido problems, normal-range estradiol occurred slightly more often than elevated estradiol. Gynecomastia was rarely documented by physicians (under 1%), even though breast tenderness was observed clinically more often — suggesting a gap between what patients actually feel and what makes it into a formal diagnosis.

Symptoms and their likelihood of a real link to high estradiol

Step 1: A blood test, not an internet guess

Before changing anything about your therapy, the first, non-negotiable step is an actual blood estradiol result — not how you feel, not a mirror photo, not a diagnosis someone on a forum made from a symptom description. Symptoms like nipple tenderness or water retention are a cue to order testing, not a substitute for that test.

That means a single, borderline result — especially if it's meant to be the basis for a medication decision right away — is worth verifying, ideally via LC-MS/MS ('high-sensitivity estradiol') and repeating after a few weeks, before treating it as proof of a real problem. Before treating a result as grounds for action

  • Testing done after the testosterone dose has stabilized (usually 4-8 weeks after it was set), not in the first weeks of therapy
  • If possible — LC-MS/MS instead of a standard immunoassay, especially with a borderline result
  • The result weighed against real, felt symptoms, not just a number against a reference range
  • If in doubt — repeat the test after a few weeks, ideally at the same lab using the same method

Symptoms without a test aren't enough

Nipple tenderness can also have other causes (e.g. mechanical irritation, skin changes), and water retention can also result from a high-sodium diet or kidney or heart problems. Before attributing them to estradiol, a blood test should confirm it.

Where genuinely elevated estradiol comes from

If testing does confirm elevated estradiol accompanying the symptoms, it's worth understanding where it came from — because that directly affects what makes sense to do first. First, too high a testosterone dose for that particular person — more circulating substrate means more material for aromatization, regardless of how well someone feels on that dose in other respects. Second, a higher proportion of fat tissue, especially visceral fat — the main site of peripheral aromatization in an adult man, so at the same testosterone dose, a man with a higher BMI will typically have proportionally higher estradiol than a leaner man. Third, individual variability in aromatase activity — some men simply metabolize testosterone into estrogen more efficiently than others, regardless of dose or body composition, partly due to variants of the CYP19A1 gene.

Why this distinction has practical significance

Each of these three causes points to a different first action: too high a dose points to reducing the dose or changing the injection schedule, a higher proportion of fat tissue points to weight loss, and high individual aromatase activity with an already reasonable dose and normal body composition is one of the few situations where an estrogen-modifying drug is warranted earlier than in other cases.

What to do — the right order of steps

Assuming a blood test has confirmed elevated estradiol and it's accompanied by the real symptoms described above — the order of action matters. Jumping straight to an aromatase inhibitor, skipping simpler and less risky steps, is one of the most common mistakes seen among men self-managing their therapy.

An order that makes sense

  • 1. A confirmed, ideally repeated, blood test showing elevated estradiol co-occurring with real symptoms
  • 2. A conversation with the doctor running your TRT — not a self-directed decision or advice from a forum
  • 3. Considering a change in testosterone dose or administration form (e.g. more frequent, smaller injections instead of infrequent, large ones) as the first line of action
  • 4. If relevant — working on fat loss, especially with an elevated BMI or visceral obesity
  • 5. Only once the above steps aren't enough, and symptoms and elevated results persist — considering an aromatase inhibitor, at the minimum effective dose, under a doctor's supervision and with monitoring

This order isn't arbitrary. Changing the testosterone dose or injection schedule, and reducing body fat, are reversible interventions, free of the risk of excessive estrogen suppression, and often sufficient on their own — especially when the cause was simply too high a testosterone level relative to individual tolerance, or excess fat tissue. Why it's not worth reaching for anastrozole on your own

Self-directed, unsupervised aromatase inhibitor dosing

Sourcing anastrozole, letrozole, or exemestane without a prescription and dosing it 'by feel', based on a schedule prescribed to someone else's body found on a forum, is a documented, recurring pattern in TRT and anabolic-androgenic steroid communities. The problem isn't that these drugs don't work — it's that without a baseline test, an established dose, and follow-up bloodwork, it's very easy to push estradiol down below the level the body needs. Symptoms of too-low estrogen — joint pain and stiffness, a drop rather than a rise in libido, worsened mood, unfavorable changes in lipid profile, and accelerated bone-density loss — can in practice be just as troubling as the symptoms the person originally tried to eliminate, and sometimes worse.

A 2024 qualitative analysis of illegal and counterfeit preparations used in self-directed 'post-cycle therapy' after stopping anabolic-androgenic steroids describes exactly this pattern: sourcing hormonal drugs, including aromatase inhibitors, outside the healthcare system, without diagnostics, without supervision, and without certainty about the actual contents of the purchased preparation. This risk isn't theoretical — it concerns both the mechanism of the drug itself (excessive estrogen suppression) and the quality and authenticity of the substance bought outside the legal supply chain.

Symptoms of excessive suppression are easily mistaken for 'not crashing estrogen enough'

Paradoxically, some men who feel worse after self-directed anastrozole interpret that as a signal the dose was too low, and increase it even further — deepening the problem instead of solving it. This is one reason why dosing decisions for this drug should be based on a blood test result assessed by a doctor, not on subjective feeling interpreted on one's own.

When an aromatase inhibitor is actually warranted

That doesn't mean aromatase inhibitors never have a place in running TRT. They do — but in a narrower set of situations than popular forum practice suggests: when estradiol is laboratory-confirmed as elevated (ideally by a method reliable in this concentration range), when it's accompanied by real, troubling symptoms as described earlier, and when simpler, reversible interventions — correcting the testosterone dose, changing the injection schedule, reducing fat tissue — have already been tried or are clearly not enough on their own. In that situation, the goal of treatment is to bring estradiol down to a level at which symptoms resolve, not to minimize it to zero — because the latter creates its own, well-documented set of problems.
