It's an intermediate state in which the body's cells start losing sensitivity to insulin, while the pancreas compensates by producing more of it to keep blood sugar in check. That absence of symptoms is exactly what makes prediabetes both dangerous and unusually promising as a point of intervention. Dangerous, because without a blood test it's easy to miss for years, during which the pancreas's insulin-producing cells gradually sustain damage. Promising, because unlike fully developed type 2 diabetes, the process at this stage is largely reversible — well-documented interventions can not only halt progression but bring blood sugar back down to normal values.

The scale of the problem

Estimates from the International Diabetes Federation and epidemiological data for European populations suggest prediabetes may affect as many as one in five or six adults, with an especially high share of undiagnosed cases precisely because it produces no symptoms.

How prediabetes is actually diagnosed

The American Diabetes Association (ADA), in its Standards of Care in Diabetes, defines prediabetes through three independent criteria — meeting just one is enough for a diagnosis. In clinical practice, fasting glucose and glycated hemoglobin (HbA1c) are used most often; the oral glucose tolerance test (OGTT) is more sensitive but less convenient for routine screening.

It's worth remembering that these three criteria don't always align in the same person — someone can have elevated fasting glucose with normal HbA1c, or vice versa. That's why, at borderline results, doctors sometimes order more than one test before confirming or ruling out the diagnosis. HbA1c reflects average blood sugar over roughly the past three months, making it less sensitive to a single day's fluctuation than one fasting glucose reading.
