Where did the number 10,000 even come from?

Contrary to popular belief, 10,000 steps a day doesn't come from a clinical trial or public health guideline — it's the name of a Japanese pedometer, the 'manpo-kei' (literally '10,000-step meter'), launched in the 1960s as a catchy marketing slogan, not an evidence-based recommendation.

Public health historians have repeatedly documented the marketing, not scientific, origin of this number — yet for decades it lodged itself in public consciousness as an assumed medical standard.

What one of the largest studies on the topic found

A study published in JAMA Internal Medicine, covering thousands of older women monitored with accelerometers, is one of the most frequently cited sources in this discussion. The results showed that the benefits of daily step count rise clearly already at relatively modest values and gradually flatten out well before reaching the conventional 10,000 mark.

Does the same hold for younger, more varied populations?

The Lee et al. study looked specifically at older women, which raises the question of whether a similar pattern (diminishing but still present benefits above a certain threshold) also shows up in a broader, more age-diverse population. A later meta-analysis covering more than a dozen international cohorts provided the answer, confirming the same general pattern — a significant reduction in mortality risk already at a moderate step count, with diminishing but not disappearing benefits at higher activity levels, regardless of age or country of the study population.

Available data shows that health benefits rise already at much lower values and follow a gradual, not a threshold, pattern — every additional step beyond a sedentary lifestyle matters, not just crossing a round number.

Does this mean more steps are pointless?

No — more movement still comes with additional benefits, especially cardiovascular and metabolic ones, and the exact point of 'enough' steps differs between studies, populations and age groups. The point is rather not to treat a round number as a rigid threshold for success or failure.

Step count isn't the only activity metric

Movement intensity (e.g. walking pace) and other forms of activity, like strength training, deliver benefits that step count alone doesn't fully capture — it's worth treating a pedometer as one of many tools, not the sole measure of health.

10,000 steps is a convenient, easy-to-remember number, but treating it as a rigid, scientifically established standard is an oversimplification. A much healthier approach is looking at your own starting point and gradually increasing activity, regardless of whether you end up at 6,000, 8,000 or 12,000 steps a day.

To patients who get discouraged because they 'never hit 10,000', I say the same thing every time: going from 3,000 to 6,000 steps a day matters more for your health than going from 9,000 to 10,000.
